  • Successful candidate would be commuting using our company vehicle across several sites (10-12+) and working in close collaboration with our pharmacy team at CMP
  • LPNs conduct an assessment on individuals for their health needs, and continue to monitor / follow-up with them daily in a way that respects any site-specific COVID restrictions in place (i.e. sometimes consultations may be done over the phone, while others are in-person) and collaborate with our prescribing pharmacists to start or administer prescription orders

    We are searching for an LPN who has the following skillset:
  • Authority to administer medications for opioid dependency and chronic pain, including Methadone, Suboxone, Kadian and other opioids
  • Active driver's license with a clean 3 year abstract and 3 years of vehicle insurance history.
  • Experience working with a vulnerable population
