Job description

We are seeking an LPN, RN, or experienced support worker for home care of a 15-year-old youth. This position requires total daily care with relevant experience and background in intellectual and developmental disability, seizure disorders, safe lift and transfers, g-tube changes and use, and medication administration. The candidate will bring knowledge and practical experience in a variety of mobility and daily living equipment/tasks, including, but not limited to wheelchair use, commode, lifts and transfers, bed safety (gaps, rail securing etc), knowledge of good diet for soft and minced, and thickened fluids. An asset you may bring is understanding and experience working with Métis individuals and families. Youth’s Diagnosis: Hypomelanosis of Ito & Epilepsy.

Shift Schedule

The position is 20-48 hours per month with shifts available every second Saturday and Sunday. Friday evenings are also available. Weekly / monthly hours are dependent on candidate availability to take available shifts.

If desired, you can swap shifts with support workers/nurses on the roster as agreed on. Additional shifts do arise over and above the set shifts. Shift times can be flexible based on applicant availability.

Benefits/Compensation

$22.00-$38.00/hour, commensurate with experience. Note the higher end of the scale will require an LPN or RN designation and registration.

Job Overview:The Homecare LPN will have the knowledge to assess, select, implement and evaluate a wide range of nursing interventions to ensure consistent, safe, competent, and ethical care. Partnering with other Maggie Willier Wellness Centre departments to utilize all resources and operate in a shared care model where applicable.

Duties & Responsibilities:

· Perform and contribute to the initial and ongoing assessment of assigned patients. Provide quality care and nursing in a variety of areas such as recovery plans, home care visits, and wound care (as qualified) with warmth and compassion.

· Encouraging patients to and assist them in completing their Activities of Daily Living (ADL) as noted on the Care Plan.

· Monitoring patients’ conditions, assessing their ability to function independently, and communicating patients’ symptoms to the Homecare Registered Nurse (RN), and other appropriate members of the health care team.

· Contribute to the delivery of emotional and physical comfort to patients including maintaining a clean environment.

· Complete accurate and thorough histories and maintain patient records in a confidential manner.

· Supervise, delegate, and provide guidance to the HCAs and the Home Makers.

· Participate and contribute to community engagements as related to improving the health and wellbeing of the community clients.

· Conduct home visits for assessments and other health-related matters as needed.

· Participate in prevention and promotion activities organized by the health department.

· Organizing specialist clinics may be required to meet the needs of the homecare clients.

· Fully comply with all Maggie Willier Wellness Centre & Driftpile Cree Nation policies, procedures, health safety, and PPE protocols - including the fitness for duty policy stipulating zero-tolerance for being under the influence of alcohol or drugs.

· May be required to work evenings, weekends, and travel as needed.

· Performs other duties as assigned by the RN, Nurse In Charge, or Health director.

Qualifications:

· Graduate of a recognized LPN program.

· Certified in First-aid CPR/AED, HCP, and WHMIS.

· Registered and in good standing with the College of Licensed Practical Nurses of Alberta (CLPNA).

· 2-3 years of related experience.

· Knowledgeable of Federal and Provincial Health Systems, and of pertinent health issues affecting the community.

· Proficient use of MS Office and web-based programs.

· Comprehensive knowledge of First Nations people, values, and culture.

· Demonstrate professionalism and ability to maintain confidentiality at all times.

· Valid class 5 driver's license, with clean driver abstract.

· Fluency in Cree is considered an assist.

· A Criminal & Child Intervention Record Check is required for this position.

Job Purpose

The Home Care Nurse supports health care in the community and fulfills the responsibilities of the Home Care and Community Health Nursing Program. The overall objective is to provide the community of Saulteau First Nations (SFN) with effective services that contribute to and enhance the health and well-being of the SFN’s children, elders, and families.

Duties and Responsibilities

Health Care Administration

• Prepares and maintains a list of clients’ receiving homecare services and schedules care aids, LPNs, and homemakers to carry out the home care program for the Nation.
• Ensures referrals, either from Physicians or self referrals, meet the criteria of the homecare program.
• Supervises and mentors Home Support workers, reporting to the Manager of Health for medical related updates, client review, evaluations, and planning.
• Develops and implements health care plans aimed at restoring, maintaining, and promoting health, as well as preventing illness, consistent with client needs and expectations, cultural, social, and spiritual needs.
• Conducts home visits, performing basic nursing duties including checking vital signs, blood sugar levels, wound care, charting, medications checks;
• Providing chronic and acute care and wellness services to community members, applying a structured client assessment process that includes on-going reassessment to determine client needs, family supports, and service allocation.
• Collaborates with medical professionals - chronic and acute care to ensure a structured client assessment process to meet client needs, family supports, and service allocation.
• Maintains a system of charting and data collection to carry out program monitoring, ongoing planning, reporting, and evaluation activities;
• Inputs charting into the EMR system
• Combines health sciences and clinical skills to effectively provide client care.
• Provides education and information to clients aimed at restoring, maintaining, and promoting health, as well as preventing illness, consistent with client needs and expectations, cultural, social, and spiritual needs.
• Advocates for services, supplies and access to practitioners/medical care as needed for SFN members.

Other Related Duties

· Works closely with the health department team to determine the health care needs of the community and design and deliver services to meet these needs.

Qualifications and Skills

· Licensed practical nurse (LPN) with First Aid, CPR-C.

· Experience nursing in a First Nations and rural community is an asset.

· Emergency and triage training to assess medical conditions and determine actions.

· Honest, trustworthy, respectful, and compassionate.

· Possess cultural awareness and sensitivity.

· Ability to build trust, serve as a positive role model and maintain confidentiality.

· Valid BC Driver’s License.

· Ability to pass a Criminal Records Check with a vulnerable sector clearance.

· Proof of COVID 19 vaccination status

Work Conditions

· Travel within SFN community and surrounding community.

· Work is performed primarily during the hours of 8:00 a.m. to 4:30 p.m. Monday through Thursdays. Occasional weekend work dependent on case load.

· Travel may be required within SFN community and surrounding community.
