Overview
Job Summary
We are seeking a compassionate and motivated Outreach Worker to join our team.
The Outreach Worker plays a vital role in supporting patients living with HIV, Hepatitis, and those enrolled in Opioid Agonist Therapy (OAT). This position focuses on helping patients stay connected to care, attend medical appointments, and overcome barriers to treatment.
If you are passionate about community health, enjoy working directly with people, and want to make a difference in the lives of individuals affected by chronic and infectious conditions, this role is for you.
Key Responsibilities
- Reach out to patients who have missed appointments or are at risk of falling out of care.
- Engage with patients via phone, text, or in-person visits as needed.
- Support individuals living with HIV, Hepatitis, and/or receiving OAT treatment.
- Collaborate closely with the medical and clinical team to ensure continuity of care.
- Provide reminders, encouragement, and practical support to help patients attend their visits.
- Maintain accurate and confidential documentation of all outreach activities.
- Build trust and rapport with individuals from diverse backgrounds and lived experiences.
- Participate in team discussions to improve outreach and patient engagement strategies.
Skills and Qualifications
- Previous experience in outreach, peer support, or community health work is an asset.
- Strong communication and interpersonal skills.
- Empathy and comfort working with people who use substances or face barriers to healthcare.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational and documentation abilities.
- Knowledge of (or willingness to learn about) HIV, Hepatitis, OAT, and harm reduction principles.
- Valid BC driver’s license and access to a vehicle are assets.
- Bilingual (English and another language) is considered an asset.
