Full job description

Contracted Social Worker

Anchor Rehabilitation & Support Services

This is an independent-contractor role with flexible hours and the option for virtual and/or community-based service delivery.

Role Responsibilities:

  • Provide counselling, psychosocial support, and system navigation
  • Assist with discharge planning and return-to-community transitions
  • Coordinate with healthcare providers, case managers, and family supports
  • Maintain clinical documentation according to professional standards
  • Deliver trauma-informed, culturally safe, client-centred care

You’re a strong match if you:

  • Are registered and in good standing with the College of Social Workers (MCSW)
  • Hold RSW or MSW credentials (or are eligible for registration)
  • Have experience in brain injury, mental health, community support, hospital transitions, or crisis-informed practice
  • Are comfortable working independently in a hybrid/remote model
  • Carry professional liability insurance

Licence/Certification:

  • Vulnerable Sector Check (required)
  • First Aid Certification (required)
  • Class G Licence (required)
  • Masters Degree: Social Work (required)

Work Location: Hybrid remote in Kitchener, ON (Waterloo District)
