Position Summary
The North York Toronto Health Partners (NYTHP), which is an Ontario Health Team (OHT) in North York, is working in shared purpose across our partnership to improve healthcare for North York and beyond is looking for an experienced Project Manager to support this change.
The successful candidate will be actively supporting in the implementation of various initiatives of the NYTHP and NYG such as:
- Primary care access and attachment initiatives, including IPCT expansion, centralized intake, supported attachment, and access clinics
- Alternate Level of Care (ALC) initiatives that support care transitions, aging in place, and connections to community-based services
- Chronic Disease Prevention and Management initiatives, including integrated pathways and supports for patients with complex or ongoing care needs
- Cancer screening and prevention initiatives that improve outreach, navigation, access, and follow-up for priority populations
- Community outreach, patient engagement, and equity-focused navigation strategies
- Performance measurement, reporting, and continuous quality improvement activities related to OHT, PCN, and provincial priorities
You will be working to support NYTHP members from across the partnership, as well as actively working on initiatives that support NYTHP strategic priorities to advance health equity and system integration. As Ontario Health Teams are working to re-design healthcare, you will have the ability to foster innovation and creativity in response to the changing situation and needs of both the initiatives you support, and the community we as a partnership are supporting. With these skills, you will be a part of shaping tomorrow’s healthcare today.
This position supports the Hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.
On a practical level, you will
- Drive the implementation of integrated primary care initiatives that improve access, attachment, navigation, and team-based care
- Support the transition of initiatives into sustainable operational models with clear governance, accountability, and performance structures
- Develop and maintain project infrastructure, including workplans, timelines, dashboards, RAID logs, and reporting tools
- Ensure projects are delivered on time, within scope, and within budget
- Monitor project progress, risks, issues, dependencies, and performance metrics, and escalate where appropriate
- Coordinate cross-sector activities across primary care, hospital, community, and OHT/PCN partners to support integrated care pathways
- Build and maintain strong relationships with internal and external stakeholders across partner organizations
- Lead and facilitate meetings, working groups, and cross-organizational forums, ensuring clear agendas, documentation, and follow-up
- Provide secretariat support to committees, including agenda setting, minute-taking, and action tracking
- Act as backbone support for key governance tables, including participation in the OHT Backbone team
- Lead reporting requirements to Ontario Health and the Ministry of Health, including milestone submissions and implementation updates
- Track, analyze, and report on performance metrics (e.g., QIP indicators, access, attachment, and patient outcomes)
- Analyze evaluation and public health data to inform program improvements and strategic decision-making
- Support measurement and evaluation activities across initiatives to enable data-driven planning
- Support the design and implementation of standardized workflows, onboarding processes, policies, and care pathways
- Contribute to the development of population-specific and team-based care models
- Conduct background research to support committees, strategic priorities, and program development
- Advance continuous quality improvement initiatives across programs
- Prepare presentations, briefing notes, reports, and other materials for leadership and partner engagement
- Develop content for periodic reporting and funding submissions
- Assist with grant applications and funding opportunities
- Participate on NYTHP committees, as required
Qualifications
- Knowledge of public health and health policy evaluation methods
- Project Management training or certification considered an asset
- Excellent verbal and written communication skills
- Strong organizational and time-management skills
- Ability to communicate data extracted from various systems to internal and external partners
- Strong relationship management and community engagement skills
