About Lu’ma

Job Summary

Under the direct supervision of the Lead Social Navigator, the Social Worker plays a key role in supporting individuals and families by improving access to services and resources that address the social determinants of health. Patients of Lu’ma Medical Centre (LMCS) will be referred through internal supports provided by the Social Navigation Team, as well as through external community service partnerships. The Social Worker provides advocacy, resource navigation, crisis intervention, and culturally safe support services to patients with complex social, emotional, financial, and community-based needs.

The Social Worker supports individuals and families by coordinating access to community resources and services that address the social determinants of health while providing advocacy to ensure patients receive comprehensive and culturally safe care. Reporting to the Lead Social Navigator, the Social Worker maintains confidential records, assists patients with forms and applications, collaborates closely with the interdisciplinary care team and community service providers, and supports holistic, patient-centered wellness. Daily work includes scheduled appointments, walk-in and spontaneous interactions, one-on-one support sessions, crisis response, and occasional workshops or community activities in a fast-paced and collaborative environment.

Responsibilities

· Provide advocacy and support to patients to ensure access to comprehensive and culturally safe services.

· Assist individuals and families in navigating barriers related to housing, income, healthcare, transportation, identification, and social supports.

· Support patients with applications and forms related to income assistance, housing, disability benefits, identification documents, and other government or community programs.

· Assist patients and families navigating systems such as MCFD, VCFSA, housing providers, and healthcare networks.

· Conduct one-on-one support sessions and provide crisis intervention when required.

· Connect patients to appropriate community resources, programs, and services.

· Work collaboratively with physicians, nurse practitioners, social navigators, Elders, and community agencies to coordinate patient care and support plans.

· Participate in interdisciplinary meetings, case conferences, working groups, and committees as assigned.

· Support referrals to internal and external health, social, financial, cultural, and recreational services.

· Build and maintain positive working relationships with patients, caregivers, community organizations, and service providers.

· Maintain accurate, timely, and confidential patient records in accordance with LMCS policies and professional standards.

· Document patient interactions, interventions, referrals, and follow-up activities in OSCAR EMR and other required systems.

· Prepare reports, correspondence, and other documentation as required.

· Ensure confidentiality and privacy are maintained in compliance with applicable legislation and ethical standards.

· Support and uphold culturally safe, trauma-informed, and patient-centered care practices.

· Respect Indigenous cultural teachings, protocols, and ceremonies while maintaining an open-minded and collaborative approach.

· Participate in organizational training, cultural learning opportunities, and professional development activities.

· Support practicum students, peers, and team members as appropriate.

Perform other related duties as assigned.

Qualifications

· Bachelor of Social Work (BSW) from an accredited institution.

· Registration in good standing with the BC College of Social Workers is preferred or required, depending on role requirements.

· Minimum two (2) years of experience in social work, community support, healthcare, or related settings.

· Experience and knowledge of First Nations’ Health Benefits/Blue Cross medical coverage.

· An equivalent combination of education and experience may be considered.

· Understanding of, respect for, and commitment to Indigenous cultural practices, traditions, and healing approaches.

· Knowledge of social determinants of health and available community resources and support systems.

· Understanding of Indigenous cultural safety, trauma-informed practice, and harm reduction approaches.

· Knowledge of appropriate legislative acts (e.g. Health Care Consent Act, Mental Health Act, Adult Guardianship Act, Freedom of Information and Protection of Privacy Act, the Declaration of the Rights of Indigenous Peoples Act, the United Nations Declaration on the Rights of Indigenous Peoples, the Child, Family and Community Services Act.

· Strong interpersonal, communication, conflict resolution, and organizational skills.

· Strong computer skills including proficiency in computer systems, programs, and electronic tools such as Microsoft Office 365 and Electronic Medical Records (EMR).

· Understanding of the scope of Social Work practice and the ability to recognize when referral to another healthcare professional or external service provider is required.

· Ability to maintain confidentiality and handle sensitive information respectfully.

· This position may require sitting, standing, and walking for extended periods throughout the workday. Frequent use of computers and electronic medical records.

· Work is performed in a community health and office environment with frequent interaction with patients, families, and community partners. Regular exposure to emotionally demanding situations, including crisis intervention and complex social circumstances.
