Coordinator

Hours: 35 hours per week
Term: 24-Month Fixed-Term Contract

At Anishnabe Long-Term Care Centre, we

We are seeking an experienced,
compassionate, and motivated Resident and Family Services Coordinator to join
our leadership team. This is an exciting opportunity for an individual who is
passionate about supporting seniors, advocating for residents and families, and
collaborating with an interdisciplinary team to provide exceptional long-term
care.
Position Summary
Reporting to the Executive Director, the
Resident and Family Services Coordinator is responsible for supporting
residents and their loved ones by providing psychosocial, emotional, and
transitional services throughout their stay at the Home. The successful
candidate will coordinate admissions, assist residents through life
transitions, provide family support, facilitate referrals, and serve as an
advocate and liaison between residents, families, staff, and community partners.
Working closely with nursing, physicians,
allied health professionals, and external agencies, the Coordinator will play
an integral role in promoting resident well-being, preserving dignity, and
enhancing quality of life through a holistic and resident-centred approach.
Key Responsibilities
Resident Support & Care Coordination
Provide emotional, psychosocial, and crisis
intervention support to residents.
Assess resident needs and develop
individualized support plans.
Assist residents in adjusting to illness,
cognitive changes, grief, family conflict, and other life transitions.
Promote resident independence, dignity, and
overall well-being.
Monitor resident progress and coordinate
ongoing supports.
Admissions & Family Support
Coordinate and oversee the admissions
process.
Welcome and support new residents and their
families throughout the transition into the Home.
Serve as the primary contact for families
during admission and onboarding.
Advocacy & Collaboration
Advocate for residents' rights, preferences,
and best interests.
Act as a liaison between residents,
families, staff, physicians, and external service providers.
Coordinate referrals and outsourced support
services.
Assist residents and families with access to
community resources and government programs.
Conflict Resolution
Facilitate respectful communication between
residents, families, and staff.
Assist in resolving concerns while promoting
a positive and supportive environment.
Program Evaluation & Quality Improvement
Evaluate the effectiveness of resident
support services.
Conduct resident and family engagement
initiatives.
Participate in accreditation activities and
continuous quality improvement initiatives.
Ensure compliance with applicable
legislation, standards, and organizational policies.
Qualifications
The ideal candidate will possess:
Diploma or Degree in Social Work, Human
Services, or a related field.
Minimum five (5) years of relevant
professional experience.
Experience working with seniors or within
long-term care is considered a strong asset.
Knowledge of gerontology, resident-centred
care, and community support services.
Understanding of Indigenous Peoples,
cultures, and traditions.
Excellent communication skills in both
English and French (written and spoken).
Strong interpersonal, conflict resolution,
and relationship-building skills.
Excellent organizational and time-management
abilities.
Demonstrated professionalism, discretion,
diplomacy, and sound judgment.
Ability to work independently while
contributing positively to an interdisciplinary team.
Strong computer proficiency and
documentation skills.
Working Conditions:
35 hours per week.
Occasional evening or weekend work may be
required.
Interaction with residents, families,
community partners, and interdisciplinary healthcare professionals.
Exposure to emotionally sensitive situations
requiring empathy and professionalism.
