Position Details:

  • Position Title: Social Worker
  • Position Type: Multiple Positions Available – Temporary; Part-time 0.5 FTE
  • Location: Southwood Care Centre
  • Hours of Work: 0800-1600
  • Union: Out of Scope

Position Summary:

The Social Worker provides services that ensure the resident experiences a sense of control over his/her environment through enhancing relationships between and among residents, families, staff, teams, departments, and the community. The role of the Social Worker includes individual and group interventions that promote intellectual, emotional, psychosocial, spiritual and financial well-being of the facility customers, and provides leadership in developing, implementing and evaluating programs that are meaningful and valued by the customers and the organization. Adheres to policies, procedures and standards. Ensures a safe and sanitary work environment. Works in partnership to improve the performance of individuals, teams, and the organization in providing improved customer service for residents, guests and staff.

Key Responsibilities:

  • Promote Intercare’s vision of being the “Heart of Excellence.”
  • Adheres to the professional Standards of Practice.
  • Ensures the provision of high quality resident care
  • Responsible for the provision of services/programs based on goals and expectations defined by the customer.
  • Facilitates research-based practice within the department.
  • Complete InterRai MDS assessment elements as appropriate.
  • Documents care, programs and interactions, as required, on the Resident’s health care record.
  • Other duties as required.

Qualifications/Skills/Job Requirements:

  • Bachelor or Master’s degree in Social Work is preferred.
  • Eligible for registration as a Registered Social Worker with the Alberta Association for Social Workers.
  • Minimum of two years’ work experience relevant to continuing care services is preferred.
  • Preference given to applicants with work experience related to cognitively impaired persons and their families.
  • Able to work flexible hours and to travel between/among Intercare facilities and homes/agencies/hospitals of customers, as required.
  • Able to read, write, and comprehend the English language.
  • Mentally and physically able to perform duties as outlined.
  • Demonstrates effective interpersonal communication (verbal and written), problem solving, critical thinking, analytical, and organizational skills.
  • Displays initiative, self-direction, and flexibility.
  • Demonstrates good leadership skills, including the ability to guide other personnel in acquiring skills and knowledge, promoting interdisciplinary collaboration, developing innovative approaches to practice and facilitating organizational change.
  • Demonstrates concern and interest in providing a safe and attractive environment for facility customers (i.e. residents, guests and staff).
  • Experience working in an acquired brain injury environment is an asset
