THE OPPORTUNITY – WELLNESS SERVICES PRACTITIONER.
The Wellness Services Practitioner is responsible for supporting our Wayfinder consumers to achieve the highest attainable standards of well-being consistent with the vision, mission, values, and guiding principles of Vecova.
Hours of work are Monday to Friday from 8:30 am to 3:30 pm.
WHAT YOU WILL DO
- Facilitate the connection of persons with disabilities to be actively engaged in the community through meaningful activities that increase their overall well-being. This may include activities such as volunteerism, recreational and leisure pursuits, life-long learning, and civic engagement.
- Assess the client’s employment readiness and identify employment skills and supports required to be competitively employed; assist in developing those skills.
- Support the client to explore, obtain, and maintain employment.
- Assist or provide support with personal care routines.
- Assist or provide full support to clients to maintain health, i.e. attending medical appointments, medication administration, liaising with pharmacy, education around healthy eating, and emotional support
- Provide some aspects of home maintenance and supporting individuals in their daily routines such as meal preparation, shopping, cleaning, and laundry
- Assist individuals in managing their personal affairs, such as banking and budgeting, paying bills, etc.
- Support individuals to know their rights, voice their thoughts and needs, build relationships in the community, and advocate for the services they require.
WHAT YOU BRING
We are looking to engage team members with diverse skills, experiences, and interests. These are some of the competencies and credentials we are looking for:
- Diploma and/or Degree in Community Rehabilitation, Disability Studies, or a related social science.
- Minimum of two years of experience working with persons with disabilities or an equivalent combination of education and directly related experience.
- Well-developed and effective interpersonal and communication skills (written and oral)
- Effective emergency and crisis management
- Must have a class 5 license, willing to use their vehicle for work, and have, or be willing to obtain, 2 million liability insurance.
- Willing to travel throughout the day to different locations
- Demonstrate a high standard of professionalism aligned with Vecova values
- Maintain a positive working relationship with all Vecova team members
- Represent Vecova in a professional manner
- Demonstrate a high level of initiative and enthusiasm
- Maintain confidentiality at all times
- Demonstrate a high level of administrative and IT capacity
