Join Our Team as a Nurse Navigator

As a Nurse Navigator, you will play a central role in supporting patients throughout their healthcare journey. Acting as a trusted guide and advocate, you will help patients navigate complex treatment pathways, coordinate care plans and ensure they feel informed, supported, and confident every step of the way.

Working closely with patients, healthcare providers, clinics and strategic partners, you will coordinate treatment plans and continuity of care while delivering an exceptional patient experience.

What You'll Do

  • Guide patients through every stage of their healthcare journey, from consultation and treatment planning through recovery and follow-up care
  • Coordinate appointments, medical records and care plans with healthcare providers and partners in Canada and abroad
  • Serve as the primary point of contact for patients and their families, providing personalized support, education, and advocacy
  • Collaborate with surgeons, clinics, and healthcare teams to ensure continuity of care and a seamless patient experience across borders

What You'll Bring

Required Qualifications

  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
  • Minimum 5 years of nursing experience
  • Experience in surgical, perioperative, case management, patient navigation and care coordination.
  • Strong understanding of surgical pathways, pre-operative preparation, and post-operative recovery
  • Proficiency with Microsoft Office 365 and electronic medical record (EMR) systems
  • Strong documentation and record-management skills
