The WCFHT is an innovative primary community health care team in Carp providing comprehensive care using best evidence practices for our patients.

We provide an environment conducive to wellness for patients and staff which promotes mutual respect, excellence in performance and adherence to the highest professional standards.

Position: Social Worker - 0.6-1.0 FTE

Starting ASAP

Duties:

As a member of the West Carleton Family Health Team, you will use best practices to provide social work assessment and treatment services for children, adults and families in a community medical centre setting.

  • Collaborate with the family physicians, psychologist and psychiatrist to provide appropriate individualized care
  • Work independently and effectively as part of a team
  • Provide individual, couple, family and group counselling
  • Effectively triage and manage a social work case load
  • Maintain up to date knowledge of community resources and services and facilitates referrals as appropriate
  • Provide timely written documentation in the electronic health record, using professional standards
  • Maintain client, team and organizational confidentiality
  • Utilize appropriate clinical supervision
  • Promote awareness of other services and programs within WCFHT
  • Maintain monthly and quarterly data collection
  • Participate in ongoing program evaluation
  • Participate in teaching and education of family medicine residents and students of other health related disciplines
  • Participate in staff, team and committee meetings (Mental Health Team) or task groups for program planning, development and evaluation
  • Organize and Provide mental health educational presentations or workshops to staff and to the patient population
  • Participate in the bi-monthly Ottawa and area FHT mental health providers group (FHT social workers and psychologists)
  • Demonstrate excellent interpersonal, verbal and written communication and organizational skills
  • Participate in self-directed professional development by attending professional workshops, conferences etc
  • Maintain professional standards and registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Maintain professional liability insurance

Qualifications

  • Masters of Social Work (preferred), Bachelor of Social work combined with experience may be considered
  • 3-5 years recent experience in clinical social work in a community setting preferred
  • Advanced assessment, crisis intervention and counselling skills in mental health, addictions and health psychology
  • Group work experience
  • Some experience in early childhood, adolescent and family counselling would be a significant asset
  • Knowledge of applicable legislated acts
  • Member of the OCSWSSW
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to work a flexible schedule including some evening hours
  • Ability to use Electronic medical record
  • Availability to work one evening per week as part of scheduled hours

Salary and Benefits Based on Qualifications

Starting Salary is starting at $35.00/hr + $5.25/hr in lieu of benefits (15%)
