QUALIFICATIONS


  • Grade 12 or suitable combination of relevant education and experience related to the position may be considered.
  • Minimum of one (1) year volunteer or work experience with vulnerable populations (e.g. mental health, addictions, seniors, youth depending on specific program area)
  • Minimum of one (1) year volunteer or work experience with developing and delivering activities and supporting the development of skills.
  • Province of Manitoba Class 5 Drivers License, and access to a personal vehicle to provide service within Prairie Mountain Health.
  • Demonstrated organizational skills, and the ability to work independently.
  • Demonstrated communication skills.
  • Demonstrated flexibility to facilitate changes in techniques and procedures.
  • Ability to display independent judgment.

POSITION SUMMARY:

The Community Support Worker 3 (CSW3) works within a Menatl Health and Addictions Services team. The CSW3 uses a client centered approach to help individuals maintain wellness in the community supporting the goals if the client within the principles of quality care. The CSW3 works collaboratively with clients and their families and other team members, as appropriate, to assess the need for, plan and develop programs, activities and supports that help to develop skills and maintain wellness within the community. In conjunction with the Supervisor, and co-workers, plans, develops and facilitates activities and connections to the other services in a designated environment to provide and promote healthy living initiatives throughout Prairie Mountain Health. The Community Support Worker 3 will utilize available community resources as appropriate to enhance a larger network of supports for the client.

RESPONSIBILITIES:

Overview

  • Provides a holistic approach (physical, psycho-social, spiritual) to strengthen client skills.
  • Promotes client independence, self-confidence, and dignity by encouraging them to perform their own activities of daily living.
  • Assists clients to be active participants in the management of their own environment.
  • Assists the development of a broader network of support for the client/family.
  • Conducts self in a professional, tactful, and courteous manner when interacting with staff, clients, families and volunteers.
  • In consultation with supervisor, schedules activities and programming to meet the identified client needs.
  • Assists in the orientation of new staff and students as requested
    Individualized Supports to Clients and Families
    With support of the Supervisor and other team members, the Community Support Worker:
  • Works with supervisor and/or counsellor/case manager to develop an appropriate plan of care,
  • Examines client strengths/needs to describe their level of functioning
  • Conducts appointments as per program guidelines,
  • Conducts home visits in an appropriate professional and ethical manner,
  • Connects with clients to develop a therapeutic working relationship,
  • Assists clients with individualized plans of care.
  • Assist clients to link with existing community resources
  • Others duties as assigned.
