Position Details
- Position: Education & Support Coordinator
- Status: Permanent Part-Time
- Schedule: 21 hours per week (3 days per week, 7 hours per day)
- Location: In-Person - Kingston Office
- Office Address: 1200 Princess Street, Kingston, Ontario
Position Summary
The Education & Support Coordinator is a permanent part-time position (21 hours per week) responsible for the development, coordination, and delivery of educational programs, support services, and community outreach initiatives that improve the quality of life of people living with Alzheimer's disease and other dementias, their care partners, and families.
Based out of the Alzheimer Society's Kingston office, the Education & Support Coordinator provides individual and group support, facilitates educational workshops and support groups, promotes dementia awareness within the community, and works collaboratively with healthcare providers, community agencies, volunteers, and stakeholders to ensure clients have access to appropriate resources and services.
The Education & Support Coordinator plays a key role in advancing the Society's mission through education, advocacy, community engagement, and person-centered support.
Key Responsibilities
Client and Family Support
- Provide information, education, emotional support, and resource navigation to individuals living with dementia, care partners, and family members.
- Conduct client assessments to identify needs, strengths, and appropriate supports.
- Develop individualized support plans and facilitate referrals to community services and healthcare resources.
- Facilitate support groups, caregiver education programs, and peer support opportunities.
- Maintain accurate and confidential client records and service documentation.
Education and Community Awareness
- Design, coordinate, and deliver educational programs and presentations for:
- People living with dementia
- Family caregivers and care partners
- Healthcare professionals
- Community organizations
- Students and volunteers
- Long-term care and retirement home staff
- Increase public awareness of Alzheimer's disease and related dementias through community outreach and public education initiatives.
- Develop educational materials and contribute content for newsletters, social media, and other communication channels.
- Participate in community events, health fairs, and awareness campaigns.
Program Development and Coordination
- Assist in the planning, implementation, and evaluation of education and support programs.
- Monitor program outcomes and identify opportunities for continuous improvement.
- Collaborate with staff, volunteers, and community partners to ensure effective service delivery.
- Support volunteer recruitment, orientation, and training related to education and support programming.
- Maintain current knowledge of dementia care best practices, resources, and emerging trends.
Community Partnerships
- Build and maintain strong relationships with healthcare providers, community agencies, and stakeholder organizations.
- Represent the Alzheimer Society on community committees, working groups, and collaborative initiatives.
- Promote available programs and services throughout the community and service area.
Administrative Responsibilities
- Maintain program statistics and prepare reports as required.
- Ensure all activities comply with organizational policies, privacy legislation, and professional standards.
- Participate in team meetings, strategic planning, and professional development activities.
- Perform other duties as assigned.
QualificationsEducation
- Post-secondary degree or diploma in Social Work, Gerontology, Nursing, Health Sciences, Public Health, Adult Education, Human Services, or a related field.
- Dementia Studies, Gerontology, or related certifications are considered an asset.
Experience
- Experience working with individuals living with dementia and/or their families.
- Experience facilitating support groups, educational workshops, or community programs.
- Knowledge of Alzheimer's disease, related dementias, and the impact on individuals and families.
- Experience in community development, health promotion, or client support services is preferred.
Skills and Abilities
- Strong interpersonal, communication, and active listening skills.
- Excellent presentation and facilitation abilities.
- Demonstrated empathy, professionalism, and cultural sensitivity.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong organizational, time management, and problem-solving skills.
- Ability to manage multiple priorities and maintain accurate documentation.
- Proficiency with Microsoft Office and virtual meeting platforms.
- Commitment to continuous learning and professional development.
Other Requirements
- Valid driver's license and reliable access to a vehicle.
- Ability to travel throughout the service area.
- Satisfactory Criminal Record and Vulnerable Sector Check.
