Job Summary

The Care Aide / Medication Support Worker provides mobile medication support services to multiple patients throughout the day. The role includes monitoring blood pressure, administering prescribed medications, performing medication witness duties, and traveling between patient locations in the community.

2 different shifts monday to friday and a weekend shift

Key Responsibilities

  • Travel to multiple patient locations throughout the day according to assigned schedule
  • Measure, monitor, and accurately record patient blood pressure
  • Administer prescribed medications in accordance with care plans, training, and scope of practice
  • Perform medication witness services and complete required documentation
  • Observe patients for adverse reactions or changes in condition and report concerns promptly
  • Ensure proper handling, storage, and transport of medications during visits
  • Maintain accurate, timely, and confidential records
  • Follow all safety, privacy, and organizational policies

Qualifications & Requirements

  • Care Aide / PSW / HCA certification or equivalent
  • Authorized training in medication administration and medication witness procedures
  • Valid driver’s license and ability to travel between patient locations
  • Reliable transportation (if applicable)
  • Strong time management, accuracy, and attention to detail

Physical & Other Requirements

  • Ability to travel frequently throughout the workday
  • Ability to perform basic clinical tasks such as blood pressure monitoring

Job Type: Part-time

Pay: $20.00-$30.00 per hour

Work Location: On the road
