Position Summary

The Transitional Support Coordinator supports participants within e4c’s transitional housing programs by providing daily, recovery-oriented services that promote stability, wellness, and independence. Working with individuals experiencing trauma, mental health challenges, addictions, and other complex barriers, the role facilitates life skills development, wellness programming, goal attainment, and community connection within a safe, non-judgmental environment. Using a strengths-based, trauma-informed, and integrated case management approach, the Transitional Support Coordinator collaborates with interdisciplinary teams to support participant recovery, build resilience, and foster long-term well-being.

Schedule

  • Hours and shifts will vary based on program and operational needs and may include day, evening, overnight, weekend, and holiday shifts.

Key Responsibilities

  • Support participants in achieving stability, wellness, and recovery through relationship-based, trauma-informed, and person-centered practices.
  • Facilitate life skills development, including daily living, self-care, medication management, communication, problem-solving, and healthy relationship skills.
  • Assist participants in identifying and achieving personal goals, developing routines, and connecting with community and natural supports.
  • Plan and deliver wellness, recovery, and life skills programming that promotes independence and long-term well-being.
  • Foster a safe, respectful, and engaging environment through positive role modeling, conflict resolution, boundary setting, and supportive communication.
  • Respond to crises, conduct risk assessments, support safety planning, and implement emergency response protocols as required.
  • Maintain accurate communication, documentation, medication administration, and compliance with organizational, health, safety, and accreditation standards.

Qualifications

Education

  • Degree in Human Services, Addictions, Social Work, Education or other relevant field

Certification

  • Standard First Aid and CPR
  • Suicide Intervention
  • Medication Administration
  • Food Safety
  • Training in Mental Health First Aid, Non-Violent Crisis Intervention, De-escalation, Harm Reduction, and Cultural Awareness is considered an asset.

Experience

  • 2+ years of direct service experience and/or relevant lived experience supporting individuals with complex needs.
  • Proven ability to foster a collaborative and supportive team environment.
  • Experience working in partnership with multiple stakeholders, service providers, and community partners.
  • Strong knowledge and application of recovery-oriented, person-centered, trauma-informed, strengths-based, and solution-focused practices.
  • Experience supporting behaviour change, accountability, and participant growth through relationship-based approaches.
