About the employer
GEM Health Care Group has been enhancing the lives of seniors in the communities where they live since 1979, operating nursing homes, residential care, and assisted living across Nova Scotia and Ontario.
About the role
GEM is creating a new clinical coordination role focused on ensuring residents with complex or elevated care needs receive individualized support quickly. When a facility identifies a resident who may require one-to-one attendant care or other individualized support, you coordinate the clinical assessment, document the resident’s assessed needs, arrange appropriately qualified care staff, and monitor the resident’s ongoing progress.
What you’ll do
- Partner with facility Directors of Care to identify residents whose assessed needs may require individualized or one-to-one support.
- Coordinate clinical assessments and document resident care needs.
- Prepare and submit resident-specific funding requests to the provincial funding authority, ensuring written approval is in place before services commence.
- Coordinate scheduling and onboarding of approved care support at the facility.
- Monitor service delivery and resident response, and prepare ongoing progress reporting on assessed care needs.
- Complete reassessments at required intervals and adjust support levels as resident needs change.
- Maintain complete, accurate clinical and funding documentation for every resident supported.
Required qualifications
- Current registration in good standing as an RN or LPN with the Nova Scotia College of Nursing.
- Minimum 3 years’ clinical experience in long-term care, continuing care, or a related setting.
- Demonstrated experience with resident assessment and care planning.
- Strong clinical documentation skills.
- Valid Nova Scotia driver’s licence and access to a reliable vehicle.
- Ability to travel regularly to facilities across Nova Scotia.
Preferred qualifications
- Experience with Nova Scotia Continuing Care programs and resident-specific funding processes.
- Experience with responsive behaviours, dementia care, or residents requiring enhanced supervision.
- Experience coordinating external care providers or agency staff.
- Familiarity with long-term care regulatory and documentation requirements.
Travel and location
Halifax-based with regular, mostly same-day travel to GEM facilities across Nova Scotia. Some schedule flexibility required to respond to urgent resident needs.
