The main purpose of this position

The Medical Office Assistant (MOA), Mobile Health Team plays a critical role in supporting the day-to-day operations of Support House's Mobile Health Team.

Working as part of an interdisciplinary team, the MOA provides administrative, scheduling, documentation, referral, and logistical support to ensure the efficient delivery of mobile primary care services in community settings across Halton Region.

This position supports physicians, nurse practitioners, nurses, and other team members by coordinating appointments, maintaining electronic medical records, facilitating communication with community partners, and assisting with outreach clinic operations.

The MOA works within a trauma-informed, harm reduction, health equity, and Housing First framework and supports individuals experiencing mental health challenges, substance use concerns, homelessness, housing instability, and other barriers to care.

Responsibilities

Medical Administration & EMR Management

  • Manage referrals, intake documentation, appointment bookings, and client registration.
  • Create, maintain, and update electronic client records within PS Suite and other applicable systems.
  • Upload, scan, and file referrals, laboratory reports, consultation notes, consent forms, and other clinical documents.
  • Monitor EMR tasks, inboxes, and follow-up requirements for clinical staff.
  • Ensure all documentation is accurate, timely, and compliant with organizational and legislative requirements.
  • Maintain strict confidentiality and privacy standards in accordance with PHIPA.

Client Coordination & Scheduling

  • Coordinate appointments, follow-ups, and outreach clinic schedules.
  • Confirm client appointments and provide reminders where appropriate.
  • Assist clients in accessing services and navigating referrals to internal and external providers.
  • Facilitate communication between clients, healthcare providers, hospitals, laboratories, pharmacies, specialists, and community agencies.
  • Support continuity of care through effective coordination and follow-up.

Mobile Health Team Operations

  • Provide administrative support to the Supervisor and interdisciplinary team.
  • Assist with planning and coordination of mobile clinics and community outreach activities.
  • Prepare materials, forms, equipment, and supplies required for clinic operations.
  • Help maintain inventory of clinical, outreach, and office supplies.
  • Support readiness and organization of the mobile clinic vehicle and equipment.
  • Maintain shared inboxes, calendars, files, and clinic tracking systems.

Program Support & Quality Improvement

  • Assist with service tracking, reporting, and data collection activities.
  • Support quality improvement initiatives and program evaluation activities.
  • Participate in maintaining procedural manuals, workflows, and reference materials.
  • Assist with audit preparation and data quality activities.
  • Support preparation of meeting agendas, minutes, and action-item tracking.

Collaboration & Community Partnerships

  • Work collaboratively with interdisciplinary team members and community partners.
  • Support communication and coordination with shelters, supportive housing providers, hospitals, primary care providers, and social service organizations.
  • Promote positive relationships with stakeholders and community partners.

Professional Development

  • Participate in supervision, team meetings, case conferences, and organizational training opportunities.
  • Engage in ongoing learning related to primary care, mental health, addictions, trauma-informed practice, harm reduction, and health equity.
  • Contribute to a culture of continuous improvement and learning.

Knowledge and skills necessary to be successful in this role

  • Diploma in Medical Office Administration, Health Administration, or related field.
  • Equivalent combinations of education and experience may be considered.
  • Minimum two years of experience in a healthcare, community health, social service, or related environment.
  • Experience with EMR systems, preferably PS Suite.
  • Experience supporting interdisciplinary teams.
  • Experience working with individuals experiencing homelessness, housing instability, mental health concerns, substance use concerns, or other complex needs is considered an asset.
  • Strong proficiency with Microsoft Office (Word, Excel, Outlook, Teams).
  • Strong organizational and administrative skills.
  • Excellent verbal and written communication skills.
  • Strong attention to detail and documentation practices.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Knowledge of PHIPA and privacy legislation.
  • Understanding of trauma-informed, harm reduction, and client-centred practice.
  • Demonstrated cultural humility and commitment to health equity.
  • Ability to work independently and collaboratively.

Working conditions

  • Work is conducted across office, community, and mobile clinic environments.
  • Regular travel throughout Halton Region is required.
  • Occasional evening or flexible hours may be required.
  • Valid Ontario driver's licence and reliable access to transportation required.
  • Ability to transport and move supplies and equipment required for clinic operations.
  • Ability to work in dynamic community settings serving vulnerable populations.
