RNS Health Care Services Inc. is currently seeking a highly organized and client-focused After-Hours Client Care Coordinator to join our dynamic team. In this vital administrative role, you will play a key part in ensuring seamless scheduling and coordination of home care services during evenings, weekends, and holidays. Reporting to the Coordination Supervisor, you will help maintain continuity of care for our clients and support our healthcare professionals in delivering timely, high-quality services.
About the Role:
As an After-Hours Client Care Coordinator, you will be responsible for managing and adjusting schedules outside of regular business hours. This includes receiving referrals, assigning staff, resolving scheduling issues, and providing support to clients and field staff. The position requires attention to detail, problem-solving skills, and the ability to work independently in a fast-paced, client-centered environment.

A Day-to-Day Look at the Role:· Receive and process referrals from Ontario Health atHome, private clients, and other referring parties.· Communicate directly with clients to confirm service needs and scheduling details.· Assign appropriate healthcare staff and ensure accurate documentation using AlayaCare.· Provide health care professionals with client details and travel directions.· Maintain continuity of scheduling and ensure staff and client safety.· Address and resolve day-to-day coordination issues promptly and professionally.· Report any unresolved concerns to the Coordination Supervisor and Client Services Manager · Complete and file documentation including incident reports, “book offs,” and other records.· Manage incoming faxes, secure the office, and arm the alarm system at shift end.· Perform other administrative duties as required.
What You Will Need to Succeed:
· Education: College or University degree required.
· Experience: Previous experience in community health care or client coordination is an asset.
· Skills:
o Excellent oral and written communication
o Strong interpersonal and analytical abilities
o Customer service expertise
o High attention to detail and strong time management skills
o Ability to work independently and prioritize under pressure
· Technical Proficiency: Comfortable using scheduling software (e.g., AlayaCare), and Microsoft Office.
Service Requirements:
· Vulnerable Sector Police Check (within 6 months)
· Flexibility to work varying shifts, including weekends and statutory holidays
· Must be available to work shifts falling between 1:00 PM – 9:00 PM, 7 days/week
The successful candidate will be required to work Mondays and Tuesday from 2:00 PM to 9:00 PM shift and Fridays and week-end days from 1:00 PM-.9:00 PM . Availability on holidays and as needed in the event of staff book-offs is also expected.
