Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in long-term care. This role requires a strong understanding of medical procedures, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The LPN will play a vital role in ensuring patient comfort, safety, and well-being through skilled nursing interventions and personalized care plans.

Duties

  • Administer medications and treatments accurately following physician orders and established protocols
  • Monitor patient vital signs and observe for changes in condition, reporting significant findings promptly
  • Assist with patient assessments, including wound care and catheter management
  • Provide specialized care for patients with dementia/seniors.
  • Document patient care activities thoroughly using electronic health records such as Point Click Care
  • Collaborate with physicians, nurses, therapists, and family members to develop and implement individualized care plans
  • Perform phlebotomy procedures when necessary and assist with laboratory testing and specimen collection
  • Educate patients and their families on health management and medication use
  • Ensure compliance with healthcare regulations, safety standards, and infection control protocols

Experience

  • Prior experience in long term environments is highly preferred
  • Experience in dementia care/senior care,
  • Knowledge of medical terminology and medication administration procedures is required
  • Familiarity with electronic health record systems such as PCC is desirable
  • Proven ability to handle resident care in diverse clinical situations including pain management and case management
  • Strong communication skills and the ability to work effectively within a multidisciplinary healthcare team
