Founded by a social worker, we are dedicated to ensuring that residents in long-term care and retirement homes have access to essential psychosocial support, enhancing their quality of life.

Summary

As a Social Worker at Age-Wise Solutions Inc., you will play a vital role in supporting the emotional and psychological well-being of residents and their families. This position offers the opportunity to work with a diverse caseload, addressing issues such as loss and grief, mental health, and family dynamics.

Responsibilities

  • Provide psychosocial support to residents in long-term care and retirement homes.
  • Conduct assessments to identify the needs of residents and develop appropriate intervention plans.
  • Collaborate with families and caregivers to address concerns related to mental health and family dynamics.
  • Advocate for residents’ rights and contribute to improved social conditions.
  • Participate in regular peer-to-peer support meetings with fellow professionals.
  • Engage in continuous professional development opportunities to enhance skills.

Requirements

  • Bachelor's or Master's degree in Social Work from an accredited institution.
  • Registered member of the Ontario College of Social Workers and Social Service Workers.
  • Experience in case management is essential; experience with elderly populations is an asset.
  • Strong communication skills, both verbal and written, for effective interaction with residents, families, and staff.
  • Ability to work independently as well as collaboratively within a team environment.
  • Demonstrated commitment to ongoing learning
  • Current Vulnerable Sector Check and clear 2 Step TB tests prior to start date
  • Professional and Commercial Liability Insurance
  • Experience in long-term care/ psychiatric / medical social work practice
  • Certificate in Gerontology or Dementia Care
  • MoCA Montreal – Cognitive Assessment trained and certified
  • Gentle Persuasive Approach
  • PIECES trained
  • Palliative Care
