Youth Care Worker is accountable to the Program Manager.

MANDATE:

JOB SUMMARY:
Assists individuals supported with physical, economic, vocational, recreational, social, emotional and daily life skills development with particular focus on the transition between youth and adulthood and or school support. Assists individuals supported to achieve a higher quality of life through increased independence, capacity building and the development of a personal network. Incumbents must have the ability to work within a multi-disciplinary team context, and be able to demonstrate skills in basic counselling, crisis intervention, conflict resolution and problem solving, as well as written and verbal communication skills.

Key Duties and Responsibilities:

  • Participates in assessment, goal setting and program planning. Documents and implements the plan. Provides input into the evaluation of the program.
  • Evaluates individuals’ needs and develops short term plans to meet such needs with the active participation of the individuals and their families.
  • Assist individuals to function more independently in their own homes, work places, school program and communities. Assist individuals with daily life skills, social skills and /or behaviour management. Teaches and assists individuals with activities such as grooming, basic cooking, money management, shopping, safety skills, pet care. Facilitates physical, recreational, educational, social and vocational activities.
  • Recognizes, analyzes and deals with potential emergency situations such as individuals’ aggressive behaviour to minimize potential harm to the person and/or the public. Report problems to the manager.
  • Administers medication to individuals in accordance with established policies and procedures.
  • Ensure health and safety standards are maintained.
  • Reviews and evaluates individuals’ progress and makes adjustments to programs as required. Provides feedback and support to individuals and/or their families.
  • Accompanies and/or transports individuals to activities such as appointments, shopping or leisure activities.
  • Provides written and/or verbal reports regarding individuals’ daily activities and progress. Ensures that all required documentation is complete and accurate.
  • Identifies social, economic, recreational, physical, vocational and educational services in the community that will meet individuals’ needs. Maintains liaison with other agencies, professionals, government officials and the community.
  • Models for the child/youth how to engage in activities and tasks such as, but not limited to; minor repairs of the home, lawn mowing, transit training, painting, and other such regular daily activities needed for the daily upkeep and maintenance of the home. Staff need to be able to lift a minimum of 5 lbs. Performs other related duties as required.
  • Is familiar with (through regular review) policies, procedures, individual protocols and care plans.
  • Contribute to team effectiveness through clear and direct communication and participation in staff meetings.

EDUCATION AND QUALIFICATIONS:

  • · A diploma in human/ social services or a related discipline or an acceptable combination of education and experience.

· A minimum of two years experience, knowledge/training in complex care needs, trauma-informed practice, positive behavioral support, children under 12 years of age with support needs, attachment-based parenting, relationship based interventions

· Knowledgeable of and encourages each person served to develop and expand his/her methods of communication

· Demonstrated ability to facilitate relationships and connections wherein persons served expand their personal support networks and find new and meaningful relationships.

· Proficient in Microsoft Word and Outlook

· Negative T.B test Clearance

· Food Safe Certification where required

· Criminal Record Check for Children and Vulnerable Adults (Provincial) - Clearance

· MCFD Criminal Record Check - Youth

· Clear Health Certificate

· Able to lift a minimum of 5 lbs.

· Valid First Aid and CPR certification

· Class 5 (Where required class 4 unrestricted driver’s license) ( If applicable in the location and/or house you are working you will need a vehicle, business insurance and 3 million liability)

· Clear and Current Driver’s Abstract.

Mandt Certification (Available training upon successful application)
