About Family Service Nova Scotia

Family Service Nova Scotia (FSNS) is a community-based provincial organization that has been supporting individuals, families, and communities since 1969. Supporting service users is at the centre of everything we do.

We provide a warm, welcoming, and non-judgmental environment across Nova Scotia, using a person-centred and trauma-informed approach that recognizes the unique strengths and experiences of each individual and family.

Program Overview

The Youth Outreach Plus Program (YOP+) supports youth experiencing multiple and complex needs across the social determinants of health.

This Tier 3, time-limited, and goal-focused program provides intensive case management to youth aged 12–26. YOP+ offers 24/7 on-call support through helplines and daytime program staffing, ensuring youth have access to support when they need it most.

Position Summary

The Youth Outreach Plus Worker provides intensive, youth-centred support in home, community, and office settings. This role works closely with youth, families, community partners, and internal teams to improve immediate and long-term outcomes across education, employment, health, and overall well-being.

Qualifications

  • Bachelor of Social Work (BSW) and relevant work experience
  • Eligibility for registration with the appropriate regulatory body
  • This is a designated position, and applicants must identify as a member of an equity-deserving community (e.g., African Nova Scotian, Mi’kmaq/Indigenous, racialized persons, persons with disabilities, or members of the 2SLGBTQIA+ community)

Core Responsibilities

Program Planning and Service Delivery

  • Complete initial assessments, including case planning and needs assessments
  • Deliver intensive, goal-focused support to youth in community, home, and office settings
  • Engage in family work with youth and their families
  • Build community connections through referrals and service linkages
  • Provide accompaniment and transportation as needed
  • Support youth in accessing basic needs
  • Provide supportive counselling to youth
  • Collaborate and consult with community partners
  • Gather feedback using the YOP+ outcome tool
  • Manage and utilize program budget to support youth needs
  • Facilitate bi-weekly meetings with Youth Outreach Programs to identify trends and potential referrals
  • Provide consultation to Youth Outreach workers as needed

Reflective Practice and Clinical Consultation

  • Attend monthly individual reflective supervision
  • Participate in bi-monthly group supervision
  • Attend team meetings as scheduled
  • Participate in monthly clinical supervision through the Centre for Family Based Training
  • Engage in case consultation with supervisor as needed

Administrative Responsibilities

  • Maintain accurate and up-to-date case notes
  • Complete monthly service user statistics
  • Submit reflective supervision documentation
  • Track and update program budget expenses
  • Record Youth Outreach Helpline (YOHL) calls

Organizational Support

  • Participate in committees and community partner meetings as needed
  • Attend OTP meetings as scheduled
  • Participate in Lunch & Learns and professional development opportunities

Operational Responsibilities

  • Track and submit timesheets, vacation, and expenses
  • Maintain program documentation and reporting requirements
  • Monitor and manage workload and scheduling
  • Support program operations as required

Requirements

  • Active registration and good standing with a professional regulatory body
  • Valid driver’s licence and access to a reliable vehicle
  • Ability to travel regularly within the community
  • Availability to work flexible hours, including evenings and on-call support
  • Clear Criminal Record Check and Vulnerable Sector Check
  • Maintenance of professional liability insurance
