Service Coordinator

ALC Community Transitions Program

The Service Coordinator will work with Individuals referred into the program by Acute care. The Coordinator will assess the medical and non-medical needs of clients being discharged from hospital and will lead the development of coordinated service plans that integrate clinical, community and social supports. The Coordinator will work collaboratively with acute care, home care, primary care, mental health & addictions and community partners to ensure a seamless handoff to the Plan Coordinator following transition to community. The goal of this position is to support older adults to transition home from ALC spaces to age safely in place by coordinating necessary supports before discharge.

QUALIFICATIONS, SKILLS & EXPERIENCE

  • Post-secondary education in Social Work, Gerontology, Nursing, Occupational Therapy or related field
  • Master’s Degree is preferred
  • Knowledge of issues impacting older adults designated ALC and transitioning from acute care
  • Experience in service planning, system navigation or complex care coordination required
  • Strong understanding of health, home care, and community-based systems
  • Ability to lead person-centered planning, problem solve, manage complexity, and make informed decisions within budget
  • Exemplary communication, facilitation and relationship-building skills
  • Police Information Check including Vulnerable Sector is required
  • Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
  • Experience working with older adults with complex needs is preferred
  • Experience working with diverse cultures and understanding barriers faced by seniors from diverse communities

This position is a permanent, full-time in-person (35 hours/week) position. The salary range for this position is $65,317 - $82,838 plus 100% Employer paid Health & Dental plan, and 5% matched Pension plan. Salary considerations are based on an assessment of the successful candidate’s education, skills, previous experience and other factors. Sage Seniors Association offers an outstanding work life balance with 3 weeks of vacation to start, a 35-hour workweek, and generous sick leave accruals. Sage also observes 13 statutory holidays in the calendar year.
