The Opportunity

We are looking for Prenatal & Family Care Coordinator to join our Family & Child Development Centre at CUPS.

The Prenatal & Family Care Coordinator is responsible for providing a caring, dignified and trauma-informed environment for clients as they work to provide integrated and holistic support to empower clients to achieve their goals.

Through regular interactions with clients, you will work to ensure that clients are provided access and support to basic needs, while also providing connection and opportunities for engagement in building parenting skills and capacity and strengthening family unity. You will work within CUPS service model of integrated care by operating in partnership with all other CUPS client services, linking clients and families to appropriate support both within CUPS and externally as needed.

• LOCATION: Onsite, CUPS - 1001 10 Ave SW, Calgary
• REPORTS TO: Manager, Family & Child Development
• CONTRACT TYPE: Contract (6 months)
• SCHEDULE: Full-time, Mon to Fri, 37.5 hours per week
• STARTING SALARY: $57,024.12 - $59,846.81

Key Responsibilities

• Build clients' capacity to navigate and access appropriate community-based resources that meet their needs, such as mental health, housing, physical health, income support, employment, transportation, food; facilitate these linkages with existing supports while avoiding duplication of care
• Offer support for Prenatal and Family Health clients attending CUPS who require assistance with system navigation or basic needs support
• Assist clients in defining initial strengths and needs through CUPS standardized assessment and track progress with the client via follow-up assessments to celebrate achievements and determine further needs.
• Liaise with Children’s Services or other involved community programs as needed to support the client in successful interactions and outcomes with external services
• Work in office, and in community as needed to provide education, support and feedback to prenatal and family health clients
• Ensure all required data is entered accurately into organizational databases including CUPS Integrated Care Tool, CUPS program Surveys, session notes and referrals provided
• Provide culturally aware support to CUPS clients and families, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present.
• Collaborate with the Family Development Centre team including but not limited to Child Development Specialist, Play Therapist and Speech and Language Therapist, to ensure that parenting strategies are in line with the developmental needs of the children.
• Collaborate with the Prenatal and Family health team, including but not limited to physicians, nurse practitioners, nurses, medical office assistants, and Prenatal Outreach Support Team (POST) outreach workers to ensure social determinants of health are addressed to support overall health and wellbeing.

What You Bring:

• Experience working with clients with complex needs
• Demonstrated skill working effectively within a multidisciplinary team
• Experience working with clients facing systemic barriers to service and support
• Good knowledge of culturally aware social service organizations and community supports
• Strong computer literacy in Microsoft Office Suite and virtual meeting software, with comfort and ability to learn new software efficiently
• Strong communication skills (oral and written) in English

Employment Requirements:

• Bachelor of Social Work (BSW)
• Active registration and good standing with the applicable regulatory body (e.g., Alberta College of Social Workers) an asset. Eligibility to register with the Alberta College of Social Workers will be considered.
• 3-4 years’ experience in case management, needs assessment and assisting individuals in accessing resources
• Valid Drivers License and reliable vehicle
• Satisfactory Criminal Record Check
• Child Intervention Check
• Current Adult/Child CPR and First Aid Certification
