About Harbourview Family Health Team:

Harbourview Family Health Team (HFHT) is a well-established primary care team based in Thunder Bay, dedicated to delivering high-quality, team-based care to our patients and their families. Our interdisciplinary team includes nurse practitioners (NPs), registered nurses, registered dietitians, social workers, administrative professionals, and 14 family physicians.

HFHT is currently in an exciting phase of growth and transition. Job Summary:

We are seeking qualified applicants for our core care teams which provide an integrated, team-based approach to care with a team consisting of physicians, nurse practitioner, social worker and a registered practical nurse. These teams are designed to enable patients to receive the right care at the right time. The core teams work with a specific patient panel, providing better access to and continuity of care.

The social worker is an integrated member of a core team, working to their full scope of practice to support the management of the pod’s collective patient panel. The social worker focuses on mental health assessments, case management, care coordination, and referrals to community resources. The social workers support patients by delivering psychoeducation to promote wellness, self-management, coping skills, recovery, crisis intervention, and support for mild-to-moderate mental health challenges. The social workers are an integral part of our service delivery, but are also an asset to our team and organizational culture.

Key Responsibilities:

Treatment, Management and Planning:

· Conduct psychosocial assessments through individual, family, and group counselling

· Provide case management, care coordination, and referrals to community resources

· Support patients in navigating health and social service systems

· Partner with patients to develop, monitor, and review individualized care plans

· Deliver psychoeducation to promote wellness, self-management, coping skills, recovery, and social supports

· Assess patient strengths, needs, and social determinants of health

· Provide crisis intervention and support for trauma, abuse, violence, serious illness, grief, and other life challenges

· Assess and support caregivers

· Consult with health care providers regarding patient care and resource needs

· Document assessments, interventions, referrals, and follow-up in the electronic medical record (EMR)

Program Development:

· Program development, delivery, and evaluation in collaboration with other professionals

· Participates in quality improvement and clinical projects

· Participates in interdisciplinary meetings and committees

· Professional development and self-directed learning to ensure up-to-date and evidence-based practices

· Seeks consultation and supervision as appropriate for complex clinical situations and participates in peer review and professional development in accordance with College standards

· Integrates inclusion, cultural sensitivity and human rights into programs and service delivery

System Navigation:

· Liaise with community professionals and services to meet patient needs

· Provide a link between the team and community based mental health services

· Actively encourage client participation in community programs services

Skills:

· Ability to manage clinical practice and caseload

· Effective written and oral communication skills

· Advanced decision making, problem identification, and problem-solving skills

· Strong interpersonal, conflict management, and de-escalation skills

· Strong organization and time management skills

· Effective documentation and record keeping

· The ability to work as part of a team in a rapidly changing environment

· Proficiency in the use of computer and information technology

· Dependable with regular work attendance

Qualifications:

· Baccalaureate degree in Social Work, preferred master’s degree in Social Work (MSW)

· Registration with the Ontario College of Social Workers and Social Service Workers, in good standing

· Membership in the Ontario Association of Professional Social Workers

· Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and community mental health resources

· Preferred 3 years counselling experience with knowledge of community systems and resources

· Proficiencies in assessment, psychotherapy and psychological counseling techniques for working with individuals and families

· Adequate professional standard Independent Health practitioner insurance coverage

· Experience working as part of an interdisciplinary team

· Effective working both independently and as part of an interdisciplinary team

· Proven ability to take initiative and contribute to team activities, organizational development, and quality improvement initiatives

· Demonstrates leadership within the clinical team and a commitment to supporting a collaborative, team-based care model

· Comfortable with electronic documentation and use of technology in the workplace

· Bilingualism (English/French or other languages) is an asset
