TUESDAY TO SATURDAY AFTERNOON ROTATION 3:00PM TO 11:00PM

Job Summary:

Reporting to the Site Manager, the Program Coordinator plans, implements, and oversees the day-to-day activities of the Shelter or Supportive Housing Programs. The Program Coordinator oversees case planning and participates in program development, policies and procedure formulation, program evaluation, revenue collection, accounting and budget reconciliation. People served are those experiencing or are at risk of homelessness, who may have complex physical and/or mental illness and substance use disorders. The Program Coordinator provides consultation to program partners and referral sources regarding issues relevant to client needs and issues.

1. Responsible for assigning caseloads to program staff.

2. Support program staff with completing case plan overview as required.

3. Confirming clients VAT and Supportive Housing Registry status

4. Support and coaching for staff around system navigation and complex case management.

5. Routine check-ins with staff about client caseloads and case management

6. Responding to escalated behaviour concerns.

7. Conversations with clients and external agencies about individuals re-accessing services.

8. Community Meetings

9. Community and Inter-agency referrals, intake coordination, and developing hospital discharge plans for clients in services.

10. Participates, monitors, encourages and facilitates clients’ participation in achieving case plans including promoting opportunities for cultural activities and liaison with on reserve services; provides guidance and instructions.

11. Reviews and evaluates client’s progress and makes adjustments to case plans.

12. Monitors and observes clients’ behaviour. Report problems to the supervisor.

13. Provides skill-building to clients such as life skills and social skills; models appropriate behaviour.

14. Ensures the safety of clients. Responds to emergencies in accordance with established policies and guidelines.

15. Responds to emergency situations and administers first aid and CPR as required. This includes responding to drug poisonings and other incidents.

16. Monitors clients who are using interior and exterior overdose prevention spaces and responds as needed to drug poisonings and other events.

17. Administers medication to the residents in accordance with established policies.

18. Accompanies and/or transports clients to medical appointment, counseling sessions and other activities as necessary.

19. Assists management in providing orientation, training, work direction and guidance to residence workers.

20. Maintains related records and statistics and provides reports to the supervisor as required.

21. Perform administrative and other related duties as required.

Qualifications

  • QUALIFICATIONS:

Required:

· Diploma in a human/social service related field.

· An equivalent combination of training, education and/or two years related experience

· Extensive knowledge of homelessness issues, mental illness and substance use disorders

· Mental Health First Aid

· Trauma Informed Practice traininng

· Occupational First Aid Level 1

· Basic CPR certification

· Food Safe

· Criminal Record Check for Vulnerable Sector

· Valid BC Driver’s License may be required for some programs.

· Ability to lift up to twelve (12) kilograms from the floor and ability to lift five (5) kilograms overhead,

· Meet requirements to stand for up to eight (8) hours,

· Meet requirements for sitting for up to one (1) hour

· Must be able to bend for up to 20 minutes at a time,

· Ability to walk on uneven terrain,

· Ability to walk up and down stairs

Pay: $30.95-$35.52 per hour
