About Us:

Reconnect Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.

As a Case Manager, you will work with clients to achieve safe, realistic, and reasonable goals within a complex health, social and fiscal environment. The Case Manager's primary function in this role is to work with older adults with addictions issues primarily in the long-term care system and provide these clients and the long-term care homes with quality health care and support services through the effective and efficient use of resources

What You'll Do:

You will be entrusted with a wide range of responsibilities, including:

  • Initiate intakes and conduct needs assessments;
  • Support to older adults with complex mental health, addiction and physical health needs;
  • Develop and implement plan of cares with short and long-term objectives, with ongoing monitoring until clients can be discharged from service;
  • Provide case management services which include assessment, intensive case management, long term planning and support with health and wellness and group activities;
  • Screen and respond to crisis calls initiated by clients, family members, friends, health care providers, and community agencies;
  • Provide consultation to long term care homes on clients who are living in their homes with active addictions issues;
  • Collaborates with the City of Toronto, judicial and other systems, organizations and individuals to ensure a well-coordinated service system;
  • Work closely with transitional care, acute care, emergency departments, rehabilitation centres, and other professionals, agencies and groups, to identify problems/gaps and possible solutions for service provision and improvement.

What We're Looking For:

To thrive in this exciting role, we seek candidates with:

  • University degree and/or Community College Diploma in social services or related field;
  • Minimum three years related experience in direct service delivery to adults with serious mental illness;
  • Demonstrated experience supporting individuals with mental health and substance use issues from a recovery and harm reduction approach.
  • Excellent verbal and written communications skills
  • Demonstrated case management and referral skills; and ability to effectively link clients to supportive services
  • Self-directed and independent, but also experienced in working effectively as a member of a team which includes internal and external stakeholders
  • Applied computer skills (Microsoft Office, Client Information Systems, Web browsing)
  • Fluency in a second language is a strong asset and preferred
  • Valid Driver’s license and access to a personal vehicle is required

  • Criminal background check;
  • Must be willing to work flexible hours including evenings and weekends, as required.
