Status: Permanent, part-time, 0.8 FTE

Schedule: Weekdays, some evening hours required.

Reports to: Program Manager, Primary Care Services

Location: Primary Care Network Wellness Centre at #4000, 1800 194 Avenue SE; Calgary, AB


Position Summary

The Social Worker collaborates with SCPCN internal and external medical home teams to deliver coordinated, high-quality, evidence-based social supports and services. As an integral member of the primary care team, the Social Worker identifies, assesses, and provides best-practice interventions for individuals, families, and groups across all ages and abilities. Responsibilities include assessment, follow-up, consultation, coordination of community resources, and facilitation of psychoeducational workshops. In collaboration with the Medical Home team, the Social Worker plays a key role in supporting patients with complex social factors—such as housing instability, food insecurity, financial hardship, mental health concerns, or social isolation—by providing comprehensive social support and services. The Social Worker also makes appropriate referrals to external agencies, community partners, and specialized service providers to ensure patients receive the resources and assistance required for their well-being. Strong communication and collaboration with the Medical Home and community partners are essential to improving patient outcomes and community well-being.

What Does the Role Do?

Patient Care
  • Conducts interviews and assessments of biopsychosocial factors affecting health and well-being, including intake and triage as required.
  • Provides social supports and services to individuals, couples, and families in collaboration with the Medical Home, and other members of the patient’s circle of care.
  • Manages a panel of patients to plan, implement, and monitor evidence-based interventions and services for patients of all ages, stages and abilities.
  • Determines and initiates referrals to internal and external providers and services as appropriate.
  • Maintains accurate and timely documentation in patient records.
  • Provides support to individuals and families in coordination with the Primary Care team, Clinical Lead and Clinical Supervisor.
  • Offers consultation and supportive referrals to physicians and other health team members, maintaining a patient-centred approach. Collaborates with other health care providers as necessary.
  • Facilitate psychoeducational workshop sessions and consultations for individuals and groups as needed.

General Duties
  • Researching current community resources, maintaining an accurate and up to date resource list, and sharing relevant information with the Medical Home and Primary Care team.
  • Engage in ongoing professional development activities and attend regular individual supervision sessions with the Clinical Supervisor to ensure continuous growth and adherence to best practices.
  • Contribute to team meetings and in conjunction with Clinical Lead, participate in program development, evaluation, and statistical reporting, and actively support quality improvement initiatives within the Social Work department.
  • Provide supervision and mentorship to practicum students, as required.
  • Occasionally perform triage responsibilities and offer support for other program areas as necessary, ensuring efficient patient care and flow.
  • Carry out additional assignments and responsibilities as delegated, maintaining flexibility and professionalism in response to organizational needs.

Who Should Apply?
  • Bachelor’s degree in social work.
  • Active registration with the Alberta College of Social Workers (ACSW) in good standing.
  • Minimum of 4 years’ experience working in an acute, outpatient and/or community mental health setting, or an equivalent combination of education, training and experience.
  • Experience working in primary care and/or knowledge of primary health care reform preferred.
  • Experience facilitating individual and group sessions is an asset.

  • Strong relationship building skills and ability to function effectively as a member of an interprofessional team.
  • Strong multi-tasking, prioritization, time management and organizational skills, with high attention to details.
  • Positive, professional attitude.
  • Proficient with computers and technology; EMR experience preferred.

Working Conditions
  • Normal office environment and remote work.
  • Evening hours required one day a week.
  • Two supervisory references required.
  • Criminal and vulnerable sector background check required.
