Clinical Pharmacist

Part-time, Fixed-Term Contract opportunity (Mon/Wed/Fri -day schedule up to 20 hours per week)

Job Overview
Get Well Family Health Team is seeking a compassionate and motivated Registered Pharmacist to join our interdisciplinary healthcare team. The Registered Pharmacist will provide both clinical and community pharmacy services, working collaboratively with physicians, nurse practitioners, and other healthcare professionals. The role includes designing and implementing health programs, providing patient consultations, and offering business guidance on establishing and operating a community pharmacy.

The pharmacist also participates in the design and delivery of health programs such as Diabetes Education, Cognitive Behavioural Therapy (CBT), Motivational Interviewing, and weight management program.

Key Responsibilities

• Provide evidence-based medication management and clinical consultation to patients and healthcare providers.
• Conduct comprehensive medication reviews to assess therapeutic effectiveness, identify drug-related problems, and recommend appropriate changesPrescribe within the scope of practice and provide counseling on the purpose, benefits, side effects, and safe use of prescribed and over-the-counter medications, supplements, and herbal products.
• Support chronic disease management programs (e.g., diabetes, hypertension, hyperlipidemia) and lifestyle management programs.
• Collaborate with the interprofessional team to develop individualized care plans tailored to patients’ health goals.
• Participate in program planning, development, and quality improvement initiatives within the Family Health Team.
• Maintain accurate documentation in EMR systems and other organizational records
• Produce reports required for patient care, organizational reporting, and quality improvement.
• Design, manage, and deliver health programs and educational sessions for patients and staff, including presentations and website articles
• Provide business consultation for establishing and managing a community pharmacy
• Communicate effectively and professionally with patients, families, and healthcare team members.
• Participate in intake, triage, and chronic disease management services
• Maintain clinic spaces, ensure compliance with Policies, Procedures, and Code of Conduct.
• Be willing to expand scope of work and train others..

Qualifications

• Bachelor’s Degree (ie. B.Sc. in Pharmacy) or Doctor of Pharmacy (PharmD)
• Registered and in good standing with the Ontario College of Pharmacists (Part A)
• Minimum 5 years of experience as a pharmacist
• Experience owning or operating a community pharmacy within the last 1 year
• Experience working closely with family doctors and primary care teams
• Experience with administering vaccinations in a team-based primary care clinic
• Ability to work both on-location and virtually
• Experience with minor ailment prescribing
• Strong knowledge of pharmacotherapy, chronic disease management, and interprofessional care
• Certifications such as Certified Diabetes Educator (CDE) or Board Certified Geriatric Pharmacist (BCGP) are considered assets
• Experience with EMR systems and proficiency in a second language (e.g., Mandarin, Cantonese, Korean, Farsi, Tagalog, Spanish) are assets
