Nurse Practitioner – Primary Care Connect Clinic
(Up to 16 hours/week, Contract to March 31, 2027)
Who We are:
The Cambridge North Dumfries Ontario Health Team (CND OHT) comprises 40+ agencies and health services providers from Cambridge, North Dumfries and the Waterloo Region. Langs’ is the host organization for CND OHT employees and so the successful candidate will be an employee of Langs.
The CND OHT in collaboration with Langs is creating a Primary Care Connect Clinic with the mandate to provide short term support to patients who are not currently attached to Primary Care. The clinic will prepare patients for attachment, and facilitate successful transitions to a permanent primary care provider within the Cambridge North Dumfries primary care network.
What We Are Looking For:
We are currently recruiting for Nurse Practitioners to join this team for a part time, casual positions. This role is responsible for working to full scope to assess, diagnose, and manage patient health needs, with a focus on stabilizing urgent or unmanaged conditions, initiating care plans, and addressing preventive and chronic disease needs. The Nurse Practitioner plays a key role in preparing patients for successful attachment to a longitudinal primary care provider, ensuring appropriate investigations, care planning, and referrals are completed so that patients are ready to transition into ongoing primary care.
A successful candidate in this role would have
· Baccalaureate in Nursing, Primary Health Care Nurse Practitioner Certificate
· College of Nurses of Ontario Certificate of Competence with RN (EC) designation (unrestricted)
· Additional academic preparation in health or social sciences considered an asset
· Commitment to the use of determinants of health model of care
· A clear understanding of local community resources
· Minimum 2 years’ experience as an NP in a primary care setting
· Experience working with individuals and families in all developmental stages
· Experience working as a member of a multidisciplinary team
· Experience in community development, prevention programming and health promotion
· Experience with Chronic Disease Management
· Experience in program development, implementation, monitoring and evaluation.
· Experience working with individual and group members of diverse populations
· Excellent communication and organizational skills
· Proficiency in the use of computers and various software applications.
In this role you will be expected to complete the following tasks
- Provide primary care services, including assessment, diagnosis, treatment, and follow-up of acute and chronic conditions
- Order and interpret diagnostic tests, prescribe medications, and perform procedures within scope of practice
- Develop and implement care plans in collaboration with physicians and interprofessional team members
- Support attachment of unattached patients and continuity of care
- Provide health education, disease prevention, and self-management support
- Participate in quality improvement initiatives and program development
- Maintain accurate and timely clinical documentation
- Contribute to a trauma-informed, culturally safe, and patient-centered care environment
- To complete other duties as required

The successful candidate would be eligible for the following benefits pro-rated to the hours of work:
· Salary of $52.89-$63.02 Hourly
· 5 Sick/Personal Days per year (pro-rated to hours worked)
· Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
