Position summary:
As a member of an integrated palliative care team, the Licensed Practical Nurse (LPN) provides compassionate, resident-centered palliative care at Maison Albert House. This role involves delivering hands-on nursing care, administering medications, and supporting residents and their families through the end-of-life journey. The LPN collaborates with the interdisciplinary team ensuring the highest standards of palliative care are delivered with compassion and respect.
Key responsibilities:
Care Delivery
- Provide direct patient care, including medication administration and personal care, while maintaining dignity and comfort for residents.
- Monitor and assess residents’ physical, emotional, and psychosocial well-being, reporting changes to clinical leadership team as needed.
- Educate residents and families about symptom management, end-of-life care and what to expect, and available support services.
- Document resident conditions and progress in the electronic medical record system (InfoAnywhere) and communicate any necessary changes.
Clinical Coordination
- Maintain accurate and timely documentation in compliance with hospice policies and healthcare regulations.
- Handle inquiries from healthcare providers, pharmacies, and family members, responding promptly and professionally.
- Address resident and family concerns with compassion, escalating to the supervisor when needed.
- Collaborate with physicians, the Director of Care, LPNs, PSWs, and healthcare providers to ensure continuity and quality of care.
Collaboration & Communication
- Foster open communication with staff, physicians, residents, and families, creating a positive team environment.
- Offer emotional support to families during the end-of-life process.
- Build strong, collaborative relationships with the healthcare team to enhance care delivery.
Compliance & Professional Development
- Adhere to hospice policies, procedures, and regulatory guidelines related to palliative care.
- Participate in training sessions, staff meetings, and professional development opportunities.
- Contribute to a culture of excellence and continuous improvement in hospice care.
Qualifications & Competencies:
- Current LPN license in good standing with the Association of New Brunswick Licensed Practical Nurses (ANBLPN)
- Fluent in English and French (ESSENTIAL)
- Experience in palliative, hospice, or long-term care is an asset.
- Strong clinical assessment and problem-solving skills.
- Excellent communication, interpersonal, and team-building skills, with a compassionate and caring attitude.
- Patient-centered approach to care and community relations.
- Ability to manage multiple priorities in a fast-paced, emotionally sensitive environment.
- Proficiency in Microsoft Office (Word, PowerPoint, Excel, and Outlook).
- Satisfactory Criminal and Vulnerable Sector Check
- First Aid/CPR Certification
