Position Summary:

The Case Aide supports the Case Management team in delivering client-centered services by assisting with administrative tasks, client support, coordination of services, and documentation. The Case Aide plays a key role in helping ensure families receive timely, consistent, and professional support, in alignment with the mission and vision of Makoon Transition Inc.

Responsibilities:

Client Support & Engagement

  • Assist Case Managers in engaging with families to support case planning and follow-up.
  • Participate in family check-ins, providing information or resources as directed.
  • Help coordinate appointments, transportation, and referrals to services.
  • Respond to client inquiries with empathy, professionalism, and discretion.
  • Support the overall wellbeing and safety of families by reporting any concerns immediately to Case Management.

Administrative & Documentation Support

  • Prepare, organize, and file client records in accordance with confidentiality standards.
  • Assist in gathering required documents from clients (IDs, forms, referrals, etc.).
  • Input and update case notes and data into internal systems as directed.
  • Support the Case Management team with case file audits and compliance tracking.
  • Maintain accurate logs of client interactions, appointments, and outcomes

Program Support

  • Coordinate logistics for meetings, case conferences, and family support events.
  • Support the implementation of case plans under the supervision of a Case Manager.
  • Assist with internal communications and tracking case timelines or deadlines.
  • Help ensure that services and supports align with cultural safety and trauma-informed practices.
  • Participate in team meetings and contribute to problem-solving and case coordination.

Safety & Professionalism

  • Maintain professional boundaries with families, staff, and community partners.
  • Follow all safety protocols and organizational policies.
  • Ensure a clean, welcoming, and organized environment in shared client spaces.
  • Maintain confidentiality and privacy of all client information.
  • Act as a positive role model, promoting the values and culture of Makoon Transition Inc.
  • Respond to family’s questions or concerns with empathy and professionalism.
  • Maintain a professional relationship with all families, child and family services agencies, community organizations and help branches.
  • MUST have your own transportation to and from work. Drive clients on caseload as needed or if Family Support Worker are unavailable.
  • Do not contribute or start Lateral Violence.
  • Participate in debriefs daily, monthly staff meetings, family events, team building activities and clinical supervision with Case Manager Supervisor and residents meeting.
  • Other duties as assigned.

Qualification:

  • High school diploma or equivalent required.
  • Post-secondary education in Social Work, Child & Youth Care, or related fields.
  • 1–2 years of experience in a social services or child welfare setting..
  • Strong interpersonal, communication, and organizational skills.
  • Ability to handle sensitive information with confidentiality and professionalism.
  • Knowledge of Microsoft Office and Google Workspace; ability to learn internal database systems.
  • Valid Class 5 Driver’s License and access to a vehicle preferred.
  • Current First Aid/CPR, Mental Health First Aid, and Non-Violent Crisis Intervention are assets.
  • Must pass a Criminal Record Check and Child and Adult Abuse Registry Check.
  • Must agree to random drug and alcohol testing to ensure workplace safety.
