,Job Overview
We are seeking dedicated RNs to join our Home Health team in Honolulu, Hawaii. This opportunity offers the chance to provide compassionate, patient-centered care in a home and community setting. Ideal candidates will have a strong background in in-home care and treatment. The role involves delivering comprehensive nursing assessments, administering treatments, and coordinating care plans to improve patient outcomes while adhering to all regulatory standards. This position is suitable for nurses committed to making a meaningful difference in patients' lives through personalized home health services.

Requirements

  • Canadian RN license (must be RN - LPN/RPN registration does not qualify)
  • NCLEX
  • Home Care or Medical Surgical Experience
  • Canadian citizenship (for U.S. TN visa eligibility)
  • Active RN License
  • 1-2 Years Minimum RN Experience

Duties

  • Conduct thorough patient assessments including vital signs, physical examinations, and medical history documentation, using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks
  • Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and HIPAA regulations
  • Manage complex medical procedures including wound care, suturing, ostomy care, tube feeding, ventilator management, and dialysis treatments
  • Perform diagnostic evaluations such as laboratory specimen collection/processing, phlebotomy, and diagnostic imaging support when applicable
  • Provide specialized care for patients with conditions like Alzheimer's disease, dementia, traumatic injuries (Level I & II trauma), infectious diseases, or post-acute recovery needs
  • Educate patients and families on health management strategies, including pain management, nutrition counseling, health coaching, discharge planning, and medication administration
  • Collaborate with multidisciplinary teams to coordinate case management and ensure continuity of care across settings
  • Maintain accurate medical documentation compliant with medical records standards and ICD coding (ICD-9/ICD-10)
  • Ensure infection control protocols are followed during all patient interactions and procedures
