Registered Practical Nurse (RPN) - Primary Health Care Full-time, Permanent (35 hours per week)$51,408 to $60,898 per annum plus benefits (salary commensurate on experience)(Filling of Vacancy)

We are seeking a candidate for the position of Registered Practical Nurse to join the Primary Health Care team based at our Lawrence Heights site. The Registered Practical Nurse works within a multidisciplinary team to provide nursing services, co-ordinate care for clients, perform certain laboratory procedures, maintain equipment and supplies, records management, and administrative support.

Job-Specific Responsibilities:

· Co-ordinate care for clients as delegated.

· Perform triage and triage support for Medical Secretary.

· Provide support to other providers through obtaining vital signs, height and weight, visual screenings, and urine checks for clients prior to their visit with clinician.

· Book ordered diagnostic tests and appointments with specialists, assist when necessary with the maintenance of recall system, communicating the appropriate information with the clients and documenting in the client’s chart where appropriate.

· Assist with administrative clinical follow-up such as calling clients with test results and requesting follow-up appointments as required.

· Assist with ensuring exam rooms, medical supplies cupboards, labs and clinicians’ offices are kept stocked.

· Order vaccines and ensure vaccine supplies are kept stocked and storage meets Public Health standards as it pertains to maintenance of the cold chain.

· Ensure exam rooms, medication and medical supplies cupboards, labs and clinicians’ offices are cleaned and maintained to the established standard.

· Maintain the availability of sterile clinical equipment by autoclaving as necessary.

· Assist with the maintenance and ordering of equipment and supplies as necessary.

· Maintain complete, accurate and timely client records and encounter data, participate in chart and case reviews and support client education.

· Other duties as assigned.

Job-Specific Qualifications:

· Registration as a Registered Practical Nurse and membership in good standing with the College of Nurses of Ontario with no restrictions to practice.

· Demonstrated experience working as a Registered Practical Nurse in a community and/or public health setting.

· 1-2 years experience in a community-based setting.

· Demonstrated expertise in medical laboratory procedures.

· Thorough knowledge and understanding of medical and laboratory terminology, medical record procedures and charting, universal precautions and other current health and safety hazards.

· Demonstrated ability to work effectively independently and as a member of a multi-disciplinary team.

· Ability to work within a community-based context, with a culturally diverse community.

· Experience with Electronic Health Records (experience in Practice Solutions Suite/PSS is an asset).  

· Familiarity with the social, economic and cultural conditions of the project’s geographic area, experience in working with local service providers, community groups and networks, and an understanding of local issues and priorities are an asset.

· Experience serving Unison’s priority populations (seniors with complex needs, people with mental health & addiction issues, LGBTQ2IA+, and youth facing barriers).

· Sensitivity to the impact of social health determinants on a diverse, urban, low-income community.

· Sensitivity to and knowledge of the needs of a multicultural, multiracial community.

· Demonstrated knowledge of, and sensitivity to, the impact of social, economic, environmental and cultural issues on diverse communities.

· Knowledge of issues affecting communities facing barriers to access in an urban environment.

· Effective communication (verbal, written and presentation), interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.

· Knowledge of a second (or more) language and culture is an asset.

· Ability to work flexible hours, including one evening per week.
