Community Healthcaring KW provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.
Community Healthcaring KW works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:
Job Title: Registered Dietitian - Diabetes Education Program - 18 months
Pay: $68,770.86 - $82,857.14 per annum
Full-Time FTE 1.0 - 18 Month Fixed Term Contract
37.5 hours per week

Job Summary
The Registered Dietitian - Diabetes Education Program (DEP) provides community based nutritional and health assessments, diabetes self-management education, insulin management, nutritional care services, and behaviour change support for people living with type 2 diabetes in the Kitchener Waterloo region.
The role operates within a client-centered health team, in collaboration with CHCKW staff, Primary Care Practitioners, Endocrinologists, Community Partners and the Waterloo Community Diabetes program.
The RD participates in the development, delivery and evaluation of programs and services for CHCKW and the regional Waterloo Community Diabetes Program.
Responsibilities
1. Provide individual and group education, nutritional and health assessments, counselling and teaching, client focused nutritional plans and behaviour change support within the RD scope of practice (as determined by the College of Dietitians of Ontario).
2. Develop client-focused nutritional plans.
3. Use EMR and other relevant technology appropriately to support patients.
4. Support the implementation of client centred insulin initiation and/or ongoing diabetes management orders as authorized by referring/primary care practitioner.
5. As a CDE provide diabetes foot checks and checks for hyperlipotrophy as needed.
6.Participate in the development, implementation, monitoring and evaluation of Diabetes Education programs for individuals and client populations as determined by CHCKW and the Community Diabetes Program, Waterloo Region.
7. Work collaboratively to perform community nutrition needs assessments and identify barriers to acquiring adequate food security and facilitate and support community initiatives to improve food security.
8. Participate in the Regional diabetes program planning, delivery and evaluation.
9.Maintain accurate and up-to-date clinical notes in accordance with College guidelines and CHCKW policy.
10. Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
11. Develop and monitor operating budgets for assigned projects/programs and ensure expenditures remain within budget.
12.As a Student Supervisor, provide orientation, ongoing support, coaching and monitoring to meet the requirements of the Health Centre and of the educational institution.
13. As a Volunteer Supervisor, provide support, training, monitoring and on-going communication as needed for the volunteer to successfully fulfill their position description.
14.Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener to be determined by the Health Centre. An evening may be required.
15. Maintain a valid driver’s license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with CHCKW Policy.
16. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
17. Collect, analyze and report on data and relevant information and participate in research as determined by CHCKW.
18. Use interpreters/translators as needed and effectively.
19. Collaborate with other team members to de-escalate situations or support clients while in crisis especially in the moments until added support is available.
20. Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
21. Work in a manner that preserves privacy and confidentiality.
22. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
23. Work in a manner consistent with Health Centre Vision, Mission and Values.
24. Work in a manner consistent with the Health Centre’s policy and commitment to Diversity, Equity and Inclusion in all processes and practices.

Accountability
This position is accountable to the Manager, Health & Wellness.
Qualifications
1. Undergraduate degree in foods and nutrition from a recognized university or equivalent.
2. Dietitians of Canada accredited Dietetic Internship or equivalent approved experience or graduate degree in community nutrition.
3. Registered member in good standing with the College of Dietitians of Ontario.
4. Certified Diabetes Educator or experience in diabetes education/care with goal to obtain CDE certification.
5. Demonstrated knowledge of clinical/nutrition guidelines appropriate for clients.
6. Three years nutritional counselling experience, preferably in a community-based setting.
7. Experience in program development, implementation, monitoring and evaluation.
8. Proficiency in the use of a personal computer and software programs including Microsoft Office, email and internet.
9. Current and ongoing CPR certifications.
10. Demonstrated ability to communicate effectively in English, both written and verbally. Second language is an asset.
11. Demonstrated ability to work collaboratively.
12. Experience working effectively with culturally and economically diverse populations.
13. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
14. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
