Your Opportunity!

Reporting to the Chief Nursing Officer, the LTCF & PointClickCare Clinical Consultant provides corporate clinical leadership, education, quality oversight, data support and systems expertise to Primacare owned and managed Long-Term Care Homes. This role supports interRAI LTCF, PointClickCare (PCC), clinical documentation, data quality, regulatory compliance and quality improvement.

Key Responsibilities and Skills:

  • Provide clinical consultation and hands-on support to Long-Term Care Home leadership and clinical teams.
  • Lead and support the interRAI LTCF program, including assessment accuracy, coding, timelines, care planning and staff competency.
  • Serve as a corporate PointClickCare clinical resource, supporting workflow optimization, standardization, troubleshooting and system improvements.
  • Monitor clinical documentation, data integrity, CIHI reporting, CMI/case-mix and quality indicators.
  • Conduct audits, identify trends and gaps, and support corrective action and quality improvement plans.
  • Develop and deliver education, coaching and resources related to interRAI LTCF, PCC and clinical documentation.
  • Support system implementations, organizational change, Ministry inspections, accreditation and regulatory compliance.
  • Collaborate with Executive Directors, Directors of Care, RAI Coordinators, clinical leaders and interdisciplinary teams across Primacare Homes.
  • Strong leadership, analytical, communication, education and problem-solving skills.
  • Ability to work independently, manage multiple priorities and support teams across multiple locations.

Qualifications:

  • Registered Nurse with current College of Nurses of Ontario (CNO) registration in good standing.
  • Minimum five (5) years of nursing experience in Long-Term Care.
  • Demonstrated experience and competency with RAI/interRAI LTCF and PointClickCare.
  • Current interRAI LTCF training/certification, as applicable.
  • Strong knowledge of Ontario Long-Term Care legislation and regulatory requirements.
  • Experience with clinical documentation, assessment, care planning and clinical data.
  • Experience in quality improvement, auditing and/or clinical program implementation.
  • Corporate/multi-site LTC, nursing leadership, CIHI, CMI/case-mix, project management and PCC implementation experience are assets.
  • Bachelor of Science in Nursing or equivalent is considered an asset.
  • Ability to travel approximately 60% throughout Ontario, including occasional overnight travel.
