Position: Patient Care Navigator

At Newleaf Total Wellness Centre, we believe patients need more than appointments and paperwork. They need someone who can listen, understand what they're going through, help them make sense of their options, and guide them toward the right care.

We're looking for a compassionate, intelligent, and highly resourceful Patient Care Navigator to become a key member of our multidisciplinary healthcare team.

This is not a traditional reception or administrative role. We are seeking someone who can build trust, ask thoughtful questions, connect the dots, and help patients navigate their healthcare journey. The right person is naturally curious, enjoys solving problems, and is passionate about helping people get better.

You may come from a healthcare, rehabilitation, wellness or sales background. Perhaps you have worked as a physiotherapy assistant, kinesiologist, rehabilitation assistant, medical office professional, or in another patient-facing healthcare, or pharmaceutical or supplements sales role. We also welcome applications from internationally trained healthcare professionals who are not currently practicing in Canada, retired healthcare professionals looking for meaningful part-time work, or individuals with extensive experience in multidisciplinary healthcare environments.

Most importantly, we're looking for someone who combines strong critical thinking skills with genuine compassion and the confidence to guide patients toward appropriate care.

As a Patient Care Navigator, you'll play a central role in helping patients understand their care options, stay engaged in treatment, and achieve the best possible outcomes.

You'll make a real difference in people's lives every day, particularly those recovering from motor vehicle accidents, workplace injuries, chronic pain, and other complex health challenges.

What You'll Do

Guide Patients Through Their Recovery Journey

Meet with new and existing patients to understand their concerns, goals, challenges, and barriers to recovery. Help them feel informed, supported, and confident about their treatment plan.

Support ICBC and WorkSafe BC Patients

Provide orientation to the clinic, explain the rehabilitation process, coordinate appointments, and ensure patients understand the services available to help them recover.

Help Patients Make Informed Healthcare Decisions

Many patients aren't sure where to start or which services will benefit them most. You'll listen carefully, assess their needs, educate them about available options, and help them access the care that is most appropriate for their situation.

Support Patients with Chronic Pain and Complex Needs

Conduct brief needs assessments, identify challenges affecting recovery, coordinate services, and help patients remain engaged in their treatment plan.

Collaborate with Clinical Teams

Work closely with practitioners to support coordinated care, facilitate communication, and help ensure patients receive a seamless experience throughout their treatment journey.

Support Team Growth

Participate in team learning initiatives, contribute ideas for improving patient outcomes, and help collect patient success stories that showcase the impact of quality interdisciplinary care.

Qualifications

We are open to a variety of educational and professional backgrounds, including:

• Experience in a multidisciplinary healthcare clinic
• Medical Office Administration
• Kinesiology
• Health Sciences
• Athletic Therapy
• Physiotherapy Assistant programs
• Rehabilitation programs
• Nursing or allied health professions
• Chiropractic, Physiotherapy, or Medical training (Canadian or international)
• Other relevant healthcare, wellness, rehabilitation, or patient-centered experience

Equivalent combinations of education, healthcare knowledge, and practical experience will be considered.

Preferred Experience

• Working directly with patients, clients, or the public
• Navigating healthcare or rehabilitation systems
• Understanding injury recovery and rehabilitation
• Coordinating services across multiple practitioners
• Educating patients or clients about treatment options
• Managing sensitive conversations with empathy and professionalism
• Building relationships and helping people move from uncertainty to action

Job Types: Part-time, Permanent

Pay: $21.00-$24.00 per hour

Expected hours: 20.0 per week
