As a Registered Nurse with our team, you will:

  • Practice within an interprofessional care team, providing comprehensive nursing assessments and supporting the care of patients with acute/episodic illnesses under the direction of physicians and nurse practitioners, including through medical directives where applicable
  • Provide health promotion, disease prevention and chronic disease management support to patients, including health teaching, self-management coaching, immunizations, and clinical procedures within the RN scope of practice
  • Collaborate, as appropriate, with physicians, nurse practitioners, members of the interprofessional care team, the local hospital, medical clinics, community care systems and the surrounding community to assess, plan, implement, evaluate and coordinate a patient's plan of care
  • Support patient education, advocacy, and system navigation, and contribute to program development and quality improvement initiatives
  • Collect data as required for statistical/reporting purposes
  • Model the values and philosophy of the FHT

Education/Experience:

  • Completion of a Bachelor of Science in Nursing (BScN) from a recognized program preferred; diploma-prepared RNs with relevant primary care experience may be considered
  • Current registration with the College of Nurses of Ontario (CNO), with no practice restrictions
  • Member in good standing of the Registered Nurses' Association of Ontario (RNAO), including professional liability protection (or equivalent professional liability coverage)
  • A minimum of 2 years of nursing experience, preferably in a primary care, community, or ambulatory care setting
  • Possess and maintain current CPR certification
  • Certification in areas relevant to primary care (e.g., chronic disease management, diabetes education, foot care, smoking cessation) considered an asset

Skills/Abilities:

  • Strong nursing assessment, patient care planning, and clinical skills, including proficiency in common primary care procedures (e.g., immunizations, injections, wound care, phlebotomy, vital signs, screening)
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care nursing, with an emphasis on health promotion, risk reduction, and chronic disease management
  • Experience working within interprofessional care teams and the ability to work effectively with patients, physicians, nurse practitioners, and community partners
  • Demonstrated organization, communication, evaluation and problem-solving abilities
  • Ability to work accurately with attention to detail and to manage competing demands
  • Proficiency in the use of computer hardware/software, particularly Microsoft suite
  • Experience working within an EMR, specifically Telus/Practice Solutions (PS Suites), would be an asset

Additional Requirements:

  • A Criminal Records Check with Vulnerable Sector screening that is acceptable to the CKLFHT
  • Provision of credentials and qualifications (diploma, degree, professional membership, etc.)
  • Current and ongoing membership in good standing with the applicable college or regulatory body for your practice area (CNO, RNAO)
  • Proof of current immunization in a form acceptable to the FHT, in accordance with the most up-to-date Canadian Immunization for Healthcare Workers
  • Possess a valid driver's license, have regular access to a vehicle and be able to travel within the community
  • Legally eligible to work in Canada
