Four Counties Addiction Services Team (Fourcast) is a community-based addiction treatment service provider that strives to provide the highest quality of evidence-based addiction treatment and case management services. Fourcast offers services across 5 sites in the region. The community treatment program staff work in-office and community settings and are often co-located and integrated with other organizations to reduce barriers for those accessing services.

Four Counties Addiction Services Team (Fourcast) is pleased to partner with various Peterborough agencies, to offer services through our new Peterborough Homelessness Addiction and Recovery Team (HART) Hub. The Peterborough HART Hub is designed as a low-barrier, trauma-informed, culturally responsive, health care model that integrates primary care, mental health, substance use treatment, housing with supports, and health systems navigation.

About The Position...

We are looking for part time and relief candidates that are experienced individuals for our transitional aftercare program to work a combination the following hours:

Days, evenings, and weekends. No overnight shifts!

Fourcast’s HART Hub After Care Transitional Housing Program supports individuals exiting Wolf Street Modular Housing Program, Bed Based Addiction programs and institutions into the substance use recovery focused After Care Transitional Housing Program to supports service users on their path to independent housing. The program works from a evidenced-based substance use recovery approach and Housing First approach using a case management model that supports housing stability and recovery.

The Transitional Aftercare Addiction Worker coaches services users in developing, maintaining, and enhancing daily living skills to promote independence, stability and overall well-being. Working within a client centered, trauma-informed, and culturally responsive framework. The Transitional Aftercare Addiction worker assists service users in achieving their personal goals related to housing stability, substance use, health, social integration and community participation.

Education & Training

  • Diploma in Social Services, Addictions, Mental Health or related field or an equivalent combination of work experience and education.

Experience

  • Minimum 1-3 years’ experience working in a related position preferred, ideally with experience in addictions, homelessness, outreach, or supportive housing.
  • Demonstrated experience working with people with complex needs related to health, substance use, mental health.
  • Experience in facilitating psycho-educational and skill building groups.
  • Experience in bed based or congregate living settings.
  • Experience using evidence-based screening tools.
  • Experience in Housing Based Case Management and assertive engagement an asset.

Certifications, Licenses and Records

  • Satisfactory Vulnerable Sector Check is required
  • Valid First Aid/CPR.
  • Non-Violent Crisis Intervention (NVCI) certificate.
  • Applied Suicide Intervention Skills Training (ASIST) an asset.
  • Food Handlers certificate an asset.

Knowledge, Skills and Abilities

  • Strong understanding of recovery oriented and relapse prevention principles.
  • Commitment to recovery orientated person-centred care.
  • Respect for dignity, autonomy and cultural safety.

  • Understanding of substance use and mental health and have a demonstrated ability to identify presenting symptoms of substance use relapse and mental health.
  • Knowledge of withdrawal symptoms for various substances and understanding of withdrawal management and the impact this has on physical and mental health.
  • Knowledge of Housing First and Housing Based Case Management to support recovery from homelessness.
  • Experience in the use of evidence-based screening tools used with those experiencing substance use and mental health challenges.
