About the position:
Under the supervision of the Program Supervisor, the Lifespan Support Worker is responsible for intensive case management and systems navigation for individuals 16 years old and up have or may have FASD, and for providing case management to caregivers who support individuals who have or may have FASD. Through the coordination of the wide range of support services, the Lifespan Support Worker will strive to enhance the day-to-day living of the individual service participant through connection to services and supports throughout their lifespans, with the goal of limiting secondary disabilities/social issues commonly associated with FASD.
For caregivers, the Lifespan Support Worker is responsible for overall case management. Included in this case management is collaboration with community partners and support creating or strengthening relationships with existing natural support networks. In addition, wraparound supports are provided to assist caregivers with parenting and household management skills.
Using a person-centered strength-based approach, Lifespan Support Workers assist the caregivers they serve by advocating, educating, referring, and creating individual case plans that are service participant directed.
Key Responsibilities:
- Provide intensive case management and system navigation to individuals aged 16 years old and up who have or may have FASD.
- Provide support of Caregivers of individuals who have or may have FASD, providing education, stability support and advocacy
- Responsible for case management with advocacy, crisis intervention, assessment, education about FASD, coaching, supported referrals to community agencies and the development of an individualized support plan.
- Provide one to one support regarding daily living skills; advocacy; mentoring; supported referrals for community access; and crisis intervention services for service participants.
- Maintain all file documentation, accurate data collection and case notes.
- Work with multidisciplinary team partners with other agencies, participant family members, community resources, spiritual resources, and/or cultural resources, among others.
- Be flexible and able to travel to respond to program needs.
- Schedule is 8:30 to 4:30 Monday to Thursday, however flexibility is frequently required to meet the needs of participants.

Requirements & Qualifications:
- Degree/Diploma in Human Services and a member of good standing with applicable professional association. Equivalent combinations of education and experience may be considered.
- FASD Education/Training. Additional consideration will be given to individuals with prior experience supporting individuals with FASD and/or caregivers.
- Knowledge of existing community resources & experience in building relationships with those resources.
- Understanding of diverse cultural identities, social issues, intergenerational trauma, mental health, and addictions. Additional understanding of how individuals who have or may have FASD are impacted by these issues will be an asset.
- Strong communication, advocacy, interpersonal, teaching, assessment, and intervention skills.
- Microsoft Office abilities (SharePoint, One Drive, Word, Excel, Outlook, etc.)
- Current Standard First Aid CPR Level C Certificate or CSA Standard Z1210-17 Intermediate First Aid Certificate
- Clear Child Intervention Record Check, dated within the past six months
- Clear Police Information Check with vulnerable sector, dated within past six months.
- Valid Class 5 Driver’s License, 3-year non-commercial Driver’s Abstract, and use of reliable vehicle with $2M Third Party Liability insurance.
