What You'll Do

As our Community Nurse Manager (Branch Lead), you will:

  • Lead, coach, and inspire a team of nurses and personal support workers.
  • Conduct client consultations, nursing assessments/reassessments, supervisory visits, and quality reviews.
  • Guide families through complex healthcare decisions by understanding their goals, challenges, and existing supports, then developing thoughtful, person-centred care plans.
  • Build trusted relationships with clients, families, hospitals, physicians, discharge planners and community partners.
  • Foster employee engagement, accountability, and a positive team culture.
  • Drive quality improvement initiatives and support branch performance.
  • Develop, review and update individualized care plans.
  • Medication management, including Best Possible Medication History (BPMH) and medication reconciliation
  • Review remote monitoring alerts and act as needed
  • Provide direct client care when needed to support your team and ensure continuity of care.
  • Identify opportunities to improve client outcomes, strengthen partnerships, and support the continued growth of the branch.

Who Thrives in This Role?

We're looking for someone who:

  • Is a Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
  • Has developed strong clinical judgment and critical thinking through hands-on nursing experience.
  • Naturally builds trust and meaningful relationships.
  • Has at least three years of nursing experience and is ready to take the next step in the leadership journey.
  • Enjoys mentoring and developing others.
  • Thinks beyond the immediate problem and seeks practical, long-term solutions.
  • Is proactive, curious, and excited by new challenges.
  • Leads with compassion, integrity, and accountability.
  • Holds a valid Ontario driver's licence and has access to a reliable vehicle.
