Job Posting – Nurse Practitioner (Primary Care)
Location: 4158 Sheppard Avenue East, Scarborough, ON M1S 1T3, with outreach to shelter and community locations in Scarborough
Status: Full-time/Part-time
Hours: 37.5 hours per week, primarily weekdays, some evening flexibility as needed

About CCRIHC
The Canadian Centre for Refugee & Immigrant HealthCare (CCRIHC) is a low-barrier, community-based clinic providing essential primary and urgent care, chronic disease management, prenatal and postpartum care, women’s health, and trauma-informed mental health services to refugees, refugee claimants, undocumented residents, uninsured newcomers, and other equity-deserving communities in Scarborough and across the GTA.
Rooted in a social justice, anti-oppressive, and health equity framework, CCRIHC brings care to people where they live, work, play, and build community, including shelters and transitional housing sites. We work within an interprofessional team that includes physicians, nurse practitioners, nurses, allied health professionals, students, and dedicated volunteers.
Position overview
The Nurse Practitioner (NP) is an integral member of the CCRIHC care team and will provide comprehensive primary care to a roster of medically uninsured and under-housed clients, both onsite at the Sheppard Avenue clinic and in outreach settings (e.g., shelters and community partners). The NP will assess, diagnose, treat, and manage acute and chronic conditions, with a strong focus on health promotion, disease prevention, women’s health, mental health, and trauma-informed care.
The NP will work to the full scope of practice, in collaboration with physician partners, and will contribute to quality improvement, advocacy, and system navigation for clients facing multiple barriers to care.
Key responsibilities
- Provide comprehensive primary care, including health assessments, diagnosis, treatment, follow-up, and ongoing management of acute and chronic conditions for uninsured and under-insured clients (adults, youth, and families).
- Order, perform, and interpret appropriate laboratory tests and diagnostic imaging within the NP scope of practice, incorporating upcoming scope-of-practice changes for NPs in Ontario as applicable.
- Provide sexual and reproductive health care, including contraception counselling, STI testing and treatment, cancer screening tests, including pap tests, pregnancy options counselling, and prenatal and postpartum care in collaboration with the interprofessional team.
- Deliver trauma-informed, culturally safe, and gender-affirming care, with particular attention to clients with histories of displacement, violence, and homelessness.
- Support chronic disease management (e.g., diabetes, hypertension, COPD, mental health conditions), including medication management, lifestyle counselling, and coordination with specialists.
- Provide health promotion, disease prevention, and health education to individuals and groups (in-clinic and outreach), tailored to language, literacy, and cultural context.
- Complete required medical documentation, referrals, and forms, including specialist referrals, diagnostic requisitions, and letters supporting access to social services and immigration processes within the NP scope.
- Collaborate closely with physicians, nurses, social workers, community health workers, and settlement partners to coordinate care and system navigation for clients with complex social and medical needs.
- Participate in case conferences, outreach clinics at Scarborough shelters and community sites, and inter-agency initiatives addressing gaps in care for uninsured and refugee populations.
- Contribute to quality improvement, data collection, evaluation activities, and student supervision as appropriate to support CCRIHC’s teaching and advocacy mandate.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO) in the Extended Class as a Nurse Practitioner (Primary Health Care or Family/All Ages).
- Minimum 3 years of recent nursing experience in primary care, community health, or related settings; experience working with refugees, immigrants, non-insured, or under-housed populations is a strong asset.
- Demonstrated knowledge and experience in chronic disease management, women’s health, sexual and reproductive health, and mental health, including harm reduction approaches.
- Strong commitment to health equity, anti-oppressive practice, social determinants of health, and the rights of refugees, migrants, and undocumented residents to accessible care.
- Excellent clinical judgment, communication, and interpersonal skills, with the ability to work effectively in a fast-paced, low-barrier, interprofessional environment.
- Comfort working to the full scope of NP practice, including independent decision-making with physician support and consultation as needed.
- Proficiency with electronic medical records (EMR) and basic office software; experience with PS Suite is an asset
- Ability to work occasional evenings or weekends to support outreach or special clinics, as required.
- Fluency in additional languages commonly spoken by newcomer communities (e.g., Arabic, Dari, Pashto, Tamil, Urdu, Spanish, Tigrinya, Somali, Mandarin, Cantonese) is considered a significant asset.
