1 full-time position – 37.5 hours/week (1.0 FTE) – $35.01/hr to $42.80/hr

1 part-time position: 3 days/week – 22.5 hours/week (0.6 FTE) – $35.01/hr to $42.80/hr

Day shift: Monday through Friday, 8:30 a.m. to 5:00 p.m.

Organization Overview

The Ste-Anne Medical Center (CMSA) is a Family Health Organization (FHO) seeking a social worker with training as a mental health counselor for its PRISME (mental health) program. The incumbent will provide psychosocial and mental health counseling services to adult clients (ages 16 and older) within the CMSA’s PRISME program.

Our team currently consists of family physicians, a consulting psychiatrist, four nurse practitioners, three registered nurses (RNs), a psychologist/psychotherapist, three social workers specializing in social intervention and counseling, a dietitian, a contract orthotist-prosthetist, a contract nursing assistant specializing in foot care, and administrative support staff.

Our clinical activities focus on five clinical programs: a mental health program, a maternal and child health program, a chronic care program (diabetes, hypertension), a nutrition program, and a foot care program.

Our electronic medical records (EMR) are supported by Telus' PS Suite software, along with its OCEAN add-ons (secure messaging and appointment reminders), and videoconferencing via OTN. You can also access them remotely through our VPN.

Profile

The ideal candidate has a background in social work and mental health, as well as experience in psychosocial intervention—and ideally in counseling and psychotherapy within the community health care setting. The person is client-centered and demonstrates exceptional skills in ethics and professional practice. They demonstrate respect, leadership, openness, flexibility, and transparency.

We are seeking a professional, empathetic, self-reliant, and organized individual with excellent clinical judgment, strong interpersonal skills, and a strong interest in collaborative work and the continuous improvement of mental health services.

Job Summary

Under the direction of the Clinical Programs Director, the incumbent conducts clinical assessments, develops and monitors individualized treatment plans, intervenes in crisis situations, and provides therapeutic services tailored to clients’ needs, primarily in person and occasionally remotely.

The position also involves the rigorous maintenance of clinical records in accordance with professional standards and confidentiality requirements, as well as active participation in the continuous improvement of services, interdisciplinary work, and the development of resources and activities to promote mental health.

The candidate also serves as a mental health resource person for the team and community partners, while keeping their professional skills up to date through continuing education and participation in best clinical practices.

Qualifications

Education: Must hold a bachelor’s degree in social work.

Desirable: Holds or is in the process of obtaining a master’s degree in social work

License: Active member in good standing of the Ontario College of Social Workers and Social Work Technicians.

Language: Excellent command of French and English (spoken and written)

Experience

· Have a minimum of 2 years of experience in social work, including at least one year of experience in mental health with adult clients, or possess knowledge and skills deemed equivalent.

Technical Knowledge and Skills

· Excellent communication skills in French and English;

· Ability to work in an interdisciplinary team and maintain good interpersonal and professional relationships;

· Ability to manage complex situations and respond in crisis situations;

· Ability to manage a patient caseload in accordance with the Program’s standards;

· Ability to work independently within established professional and organizational boundaries;

· Strong organizational, planning, and prioritization skills for handling concurrent emergencies;

· Flexibility and the ability to adapt to circumstances;

· Proficiency with technology and electronic medical records.
