Rehab First Inc. is currently seeking a Social Worker to join our team in the Kitchener-Waterloo area to support our growing client base through community-based service delivery. This posting is for an existing vacancy.

Rehab First is one of Ontario's largest multidisciplinary assessment and treatment organizations, with a growing team of approximately 120 professionals specializing in a variety of disciplines. Guided by our values - sincerity, commitment, and self-improvement - we aim to restore hope and function for clients impacted by serious injury and illness.

Key Responsibilities:

  • Psycho-social assessments
  • Selective assignment of Case Management responsibilities
  • Provision of counselling services in a variety of settings (in-person, via telephone, virtual counselling)
  • Individual, couple, and family counselling
  • Supporting clients with systemic issues such as housing or food insecurity
  • Connecting clients with social/community supports
  • Assist with social reintegration of clients
  • Provide client treatment in community settings
  • Work with a diverse client base
  • Prepare and submit case notes and reports
  • Collaborate effectively within a multidisciplinary team
  • Opportunity to work across the lifespan or to specialize

Qualifications:

  • Master's degree in Social Work (MSW)
  • Current registration and in good standing with the Ontario College of Social Workers and Social Service Workers
  • Experience in healthcare, ideally in the community
  • Familiarity with relevant legislation (Health Care Consent Act, Mental Health Act)
  • Valid CPR certification
  • Valid G driver's license, auto insurance, and access to a reliable vehicle
  • Satisfactory Criminal Record Check with Vulnerable Sector Screening (dated within the last 6 months prior to hire date)
  • Superior communication, problem-solving, time-management and documentation skills
  • Self-motivated and demonstrated ability to manage a varied caseload
  • Demonstrated ability to work collaboratively as a member of a multidisciplinary team
