Job Description - SOCIAL WORKER II, CIFT
Hours: 37.5 hours per week
Status: Full-Time/Permanent
Union/Non: This is a unionized position
Rate of pay: $39.88 - $43.17 per hour (based on experience)
This posting is for a replacement position within our organization.
Position Summary
As the ideal candidate for this position, this individual must be able to provide evidenced based practices for individual and family counselling sessions as well as group counselling sessions, must manage their own caseload including clinical documentation, must facilitate all plan of cares and ensure all interdisciplinary team members have their sections completed appropriately, will be responsible for setting discharge dates within 3 to 6 months of admission and work with the teams to ensure successful transition back to the community within that time frame, administer standardized tools and use the results in treatment planning (tools include the INTER-RAI, CALOCUS, PHQ9, and GAD 7), and demonstrate flexibility and be self-motivated. This individual will also possess excellent organizational, communication and interpersonal skills as well as the ability to work effectively within a dynamic, multidisciplinary team environment.

This position reports directly to the Director, Quality & Inter-Professional Practice
Supervises: Social Worker Students
Corporate Responsibilities
- Fosters positive interpersonal relationships with the staff, residents, and visitors, outside agencies, government officials and related professional associations.
- Maintains and adheres to organizational policies and procedures.
- Ensures Legislation requirements around documentation is being followed, including Plan of Care standards.
- Attends work on a regular and consistent basis.
- Adheres to Occupational Health and Safety Policies.
Job Responsibilities
- Conduct bio-psycho-social assessments, risk assessments, and provide evidenced based regular individual, family, and group therapy sessions with outpatient, day treatment and Live in Treatment clients.
- Modalities to be used include Trauma Focused Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and Emotion Focused Family Therapy.
- Client will receive through the CIFT staff an average of 6-12 individual counseling sessions based on these modalities, and then 6-12 family counseling sessions. CIFT will also lead evidenced based groups, including a group for caregivers
- Attend meetings in office, community and treatment residences including but not limited to school meetings, case conferences, treatment planning meetings, and psychology/psychiatric appointments. Attend administrative and clinic team meetings and trainings.
- Will be responsible to schedule, facilitate, and lead all Plan of Care meetings, and to ensure interdisciplinary members have completed their sections appropriately and on time.
- Will be responsible for ensuring the full Plan of Care packages are provided o the treatment team and caregivers 3 days prior to the Plan of Care meeting, and that all signatures are obtained at the conclusion of the meeting.
- Meet with Live in Treatment Supervisors and Day Treatment team regularly to coordinate each client’s individual treatment plans Responsible for setting discharge dates within 3 to 6 months of admission and ensuring clients are transitioned safely back to the community within that timeframe
- Complete required documentation including assessments, standardized tools, session notes, treatment plans, Serious Occurrence Reports, and disclosures to CAS s in a timely manner.
- Liaising with clients and their parents/guardians as well as other professionals such as school personnel, probation workers, and Children’s Aid workers.
- Provide intensive individual and family counselling sessions, transition/discharge support, and outreach services to ensure coordination and delivery of services to meet the needs of clients.
- Attend weekly or biweekly individual supervision with Clinical Supervisor.
- Be available to work 2-3 evenings per week.
- Other duties as assigned.
Qualifications
- A Masters of Social Work (MSW) or equivalent education is required.
- Minimum 2 years of relevant post qualification experience.
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the College of Registered Psychotherapist of Ontario (CRPO).
- Clinical experience in providing bio-psycho-social assessments, risk assessments, and evidenced based individual and family therapy to children, adolescents and/or their families.
- Clinical experience providing evidenced based group sessions to children, adolescents and/or caregivers is considered an asset.
- Excellent verbal and written communication skills.
- Experience with using a variety of evidence-based/informed treatment approaches such as TF-CBT, DBT, EFFT, and/or McMaster Approach to Family Therapy.
- Experience in working in home and/or in the client’s community preferred.
- Evening availability required.
