What We Will Ask of You:
As a key member of Quality Assurance and Specialized Services team, our Adult Case Manager (ACM) plays an important role. On most days you’ll be providing specialized case management support and oversight within the province of Ontario. You’ll be engaging with service users and service providers through regular communication and check in meetings to ensure identified goals are met and adjusted to changing support needs, administrative needs, as well as providing financial oversight of individualized funding through monitoring submissions of service user invoices and expenses. The ACM ensures the OPR supports for service users maintains multiple legislative requirements within Ministry of Children, Community and Social Services (MCCSS), Quality Assurance Measures (QAM) and Serious Occurrence reporting (SOR) requirements. The ACM collaborates with the Program Manager, Team Lead, Adult Case Managers & Transitional Age Youth Coordinators and team to ensure that evidence based best practices are followed and quality service is delivered while enhancing the overall quality of life and independence for individuals accessing services.
Primary Responsibilities:
- Provide specialized case management support and oversight to adults living with a developmental disability residing within the province of Ontario receiving individualized adult funds through Ministry of Children, Community and Social Services (MCCSS).
- Ensure all Canopy and Outside Paid Resources (OPRs) Quality Assurance Measures (QAM) activities operate within relevant legislation and best practice integration within agency adult services to achieve legislative compliance, maintain the ability to provide service delivery and achieve agency goals.
- Develop Individual Support Plans (ISP)s and attend meetings hosted by OPRs and other meetings as appropriate.
- Act as a resource and support to external staff and Program Managers regarding QAM and ACM inquiries.
- Monitor monthly expenditures and track expenditures for clients.
- Complete SORs for Canopy and all associated OPRs.
Qualifications:
- College Diploma in Social Sciences or equivalent work experience.
- 3 + years of experience of direct support experience in a residential setting.
- Registered Social Service Worker designation is considered an asset.
- Case Management experience is considered an asset.
- Working knowledge of Serious Occurrence reporting guidelines.
- Direct experience working with people with an intellectual disability or equivalent.
- Valid G-Class driver’s license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars
