Social Worker (MSW, BSW, RP)

Permanent Full-Time Position (up to 30 hours/week)

Location: Simcoe, Port Dover

About Us

The Norfolk Family Health Team (NFHT) is a community-based, not-for-profit organization committed to providing accessible, high-quality, and person-centered primary care to individuals and families in Norfolk County. Our services support individuals facing social, economic, and systemic barriers through interprofessional care teams and partnerships.

Our team includes family physicians, nurse practitioners, registered nurses, social workers, a registered dietitian, pharmacist, medical office assistants, a kinesiologist, and an occupational therapist. Additional services such as pharmacy, community lab, ultrasound, and various health and wellness programs are also offered onsite.

Position Summary

As an integral member of the interprofessional care team, the Registered Social Worker provides high-quality, short-term therapeutic support to both rostered and unattached patients. Services include assessment, brief counseling, crisis intervention, psychosocial education, and system navigation, with a focus on individuals living with chronic disease, anxiety, depression, and chronic pain.

Interventions are offered in both individual and group settings. The Social Worker collaborates with the team to support the development of care plans that reflect the goals and needs of each patient.

Key Responsibilities

  • Conduct psychosocial assessments to identify social, emotional, mental health, and environmental factors impacting patient health
  • Provide short-term, solution-focused counselling and psychotherapy within scope of practice
  • Support patients with chronic disease management, pain management, and adjustment to illness or life transitions
  • Deliver crisis intervention and safety planning when required
  • Develop collaborative, patient-centered care plans in partnership with the interprofessional team
  • Provide system navigation, advocacy, and linkage to community resources and social supports
  • Facilitate individual counselling and group programs focused on mental health, chronic disease, and wellness
  • Support patients with social determinants of health including housing, income, food security, and caregiver stress
  • Participate in case conferences and interprofessional team meetings to coordinate care
  • Maintain accurate, timely, and confidential documentation in the electronic medical record (EMR)
  • Contribute to program development, quality improvement, and evaluation initiatives
  • Support transitions of care and coordination with hospitals, community agencies, and primary care providers
  • Adhere to professional standards, ethics, and regulatory body requirements (OCSWSSW or CRPO)

Qualifications and Experience

  • Bachelor of Social Work (BSW), Master of Social Work (MSW), or registration as a Registered Psychotherapist (RP) in good standing with the appropriate regulatory college (OCSWSSW or CRPO)
  • Minimum of 2 years’ experience working in mental health, primary care, or with individuals living with chronic pain and/or chronic illness
  • Training and/or experience in crisis intervention, suicide risk assessment, and safety planning
  • Experience providing brief, evidence-based counselling or psychotherapy (within scope of practice)
  • Experience with program development and delivery, including group-based and virtual programming
  • Familiarity with program evaluation, quality improvement, and outcome measurement
  • Strong understanding of social determinants of health and community-based supports
  • Knowledge of privacy legislation (e.g., PHIPA) and referral pathways within health and social service systems
  • Strong communication, collaboration, and documentation skills in an interprofessional primary care setting
  • Ability to work effectively both independently and as part of a team-based care model
