Overview
CareOnDemand is a service provider and staffing solution company that delivers consistent and ongoing nursing, allied health, health support, and community services to clients across various care settings. As part of our Client & Staff Services Team, you will join a growing group of professionals who provide high-quality nursing care directly in clients’ homes or community environments. You will promote and support client safety, health, and independence while fostering strong relationships with clients, families, caregivers, and other healthcare partners.
Speech-Language Pathologist (SLP) Duties and Responsibilities:
Key Responsibilities
- Administer standardized and non-standardized assessments, clinical observations, & diagnostic tests to evaluate speech, language, voice, fluency, resonance, cognitive-linguistic, and swallowing disorders.
- Develop and implement individualized treatment and care plans based on client goals, assessment findings, and evidence-based practices.
- Provide 1:1 therapy session in clients’ homes, schools, or community environments, adapting techniques to meet each client’s functional needs.
- Collaborate and coordinate care with families, caregivers, and interdisciplinary team members (e.g., nurses, occupational therapists, physiotherapists, personal support workers) to ensure continuity and effectiveness of care.
- Educate and counsel clients, caregivers, and family members regarding communication strategies, swallowing safety, and home-based exercises or maintenance programs.
- Monitor and document client progress, maintaining accurate and timely clinical records in accordance with professional standards and CareOnDemand policies.
- Provide input into discharge planning and make recommendations for ongoing care or community supports as appropriate.
- Supervise and mentor Communicative Disorders Assistants (CDAs) and other health care personnel involved in the delivery of communication or swallowing therapy programs, as applicable.
- Promote a safe, respectful, and compassionate care environment in every interaction.
Speech-Language Pathologist (SLP) Skills and Qualifications
Skills & Competencies
- Strong oral communication and active listening skills to engage effectively with clients, families, and healthcare teams.
- Excellent social perceptiveness and empathy to respond to client needs and cultural considerations.
- Skilled in assessing, developing, and carrying out individualized treatment plans.
- Ability to instruct and guide caregivers or support staff in therapy implementation.
- Proficient in monitoring client progress and adjusting interventions as needed.
- Strong writing and documentation skills for accurate clinical records and reporting.
- Ability to work independently in home or community settings while maintaining safety & professional standards.
- Organized, detail-oriented, and adaptable to changing client needs and schedules.
- Proficient with electronic health records and basic computer applications.
Qualifications
- Education: Master’s degree in Speech-Language Pathology from an accredited program.
- Registration: Current registration and good standing with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO).
- Training: Demonstrated experience in assessing and treating communication, speech, language, cognitive-linguistic, and swallowing disorders; ability to develop and implement individualized treatment plans.
- Experience: Minimum 1–2 years of experience, preferably in community-based, home care, or 1:1 therapy setting.
- Screening: Current Vulnerable Sector Check.
- Physical Requirements: Ability to safely assist clients with basic mobility or positioning as needed during therapy sessions.
- Transportation: Valid driver’s license and reliable transportation to visit clients in the community.
