Full job description

Elements Support Services is seeking dedicated and compassionate Mental Health Workers to join our team in the Greater Toronto Area. In this dynamic role, you will provide essential mental health support to individuals and families across a variety of settings.

A significant focus of this position involves working directly within family homes and supporting clients as they access the community and participate in community activities. As a pivotal member of our team, your commitment to fostering independence, social inclusion, and mental health stability is essential.

Hours: Shift work, flexible hours up to 44 hours/week Settings: Family Homes, Community, Residential, and Hospital Settings

What You’ll Do

  • In-Home Support: Provide individualized, trauma-informed care directly within family homes, fostering a supportive environment for both the client and their family unit.
  • Community Integration: Facilitate and accompany clients to community activities, helping them navigate social settings and access local resources to promote inclusion and independence.
  • FASD Specialized Support: Implement specialized strategies for clients with Fetal Alcohol Spectrum Disorder (FASD), with a focus on environmental modifications and routine consistency both at home and in the community.
  • Assessment & Planning: Collaborate with multidisciplinary teams to develop care plans that address mental health, neuro-developmental needs, and community participation goals.
  • Crisis Intervention: Respond effectively to crisis situations in various environments using de-escalation strategies and safety-focused interventions.
  • Therapeutic Interventions: Utilize counseling approaches and behavior support techniques to assist clients in managing challenges and achieving personal goals.
  • Documentation: Maintain accurate, professional documentation of client progress, community engagement, and interventions.
  • Collaboration: Work closely with families and caregivers to provide psychoeducation and coordinated care strategies within the home.

Qualifications/Requirements

  • Education: Child and Youth Worker (CYW), Social Service Worker (SSW), or a diploma/degree (minimum two years) in Human Services or a related field
  • A minimum of two years of direct experience supporting individuals with mental health presentations in various settings
  • Proven experience or specialized training in supporting individuals with Fetal Alcohol Spectrum Disorder (FASD)
  • Clear Vulnerable Sector Check (within the last 6 months)
  • Current First aid & CPR
  • Current Crisis Management training (NVCI, UMAP or SMG)
  • Driver’s License: Valid driver’s license and access to a reliable personal vehicle
