Description

The Scarborough Academic Family Health Team (SAFHT) is seeking a qualified and experienced individual to join our team as a Physician Assistant (PA). SAFHT has 2 new PA roles that will report to the Executive Director and/or manager and will work our interdisciplinary team. The successful candidates will primarily work within one of SAFHT’s affiliated physician offices and collaborate closely with the supervising physician and interdisciplinary team to provide comprehensive, patient-centred primary care services. The PA may be required to travel between SAFHT sites as operationally required.

Working under the supervision of an affiliated physician and within the delegated scope of practice, the Physician Assistant provides comprehensive medical care to patients across the lifespan within a primary care setting.

The Physician Assistant collaborates with physicians and the interdisciplinary team to provide timely, accessible, evidence-informed care that supports prevention, health promotion, chronic disease management, episodic care, and continuity of care. The PA practices in accordance with Ontario legislation, Medical Directives, delegation agreements, organizational policies, and the standards established by the Canadian Association of Physician Assistants (CAPA).

About Us:

The SAFHT is governed by a Mixed Board (i.e. Physicians and Community Members) and is an integrated group of health care providers that work collaboratively as part of a family health team. Our interprofessional care team currently includes 29 affiliated physicians, nurse practitioners, registered nurses, registered dietitians, a case manager, social workers, chiropodists, and several administrative staff. Originally formed in 2007, the SAFHT serves patients by providing them with timely access to a comprehensive range of services and programs aimed at managing and preventing disease and improving health status. Including our SAFHT affiliated physician group locations, we deliver care across 8 locations in the Scarborough and Eastern Toronto geography and engage in teaching learners in medicine and other health disciplines. Role Responsibilities:

Working collaboratively with the supervising physician and interdisciplinary team, the Physician Assistant:

  • Performs comprehensive patient assessments, including obtaining medical histories and conducting physical examinations.
  • Develops differential diagnoses and proposes assessment and management plans for review with the supervising physician, where appropriate.
  • Assist with onboarding new patients
  • Orders and interprets laboratory investigations, diagnostic imaging, and other diagnostic tests as delegated.
  • Provides assessment and management of acute and episodic illnesses within delegated authority.
  • Provides ongoing assessment and management of patients with chronic diseases including diabetes, hypertension, COPD, asthma, heart failure, osteoporosis, chronic kidney disease, and other common primary care conditions.
  • Performs medical procedures within delegated authority and demonstrated competence, including wound management, injections, immunizations, suture and staple removal, ear irrigation, ECG interpretation, and other office-based procedures.
  • Provides preventive care including cancer screening, cardiovascular risk assessment, immunizations, smoking cessation support, and lifestyle counselling.
  • Perform well-baby visits
  • Perform and assist in performing pelvic examinations.
  • Provides patient and family education regarding diagnoses, treatment plans, medications, and disease prevention.
  • Provide geriatric care
  • Perform house calls when needed.
  • Prescribes medications and renews prescriptions where delegated and authorized.
  • Coordinates referrals to specialists, diagnostic services, hospitals, and community resources.
  • Reviews laboratory and diagnostic results and initiates appropriate follow-up within delegated authority.
  • Documents patient encounters accurately and completely within the TELUS Practice Solutions Suite (PSS) Electronic Medical Record.
  • Collaborates with physicians and interdisciplinary team members to develop comprehensive care plans.
  • Participates in quality improvement initiatives, clinical audits, and program evaluation activities.
  • Supports learners and contributes to SAFHT's academic and teaching environment where appropriate.
  • Maintains patient confidentiality and complies with all organizational policies, privacy legislation, and infection prevention and control standards.

Accountability

The PA reports to the Executive Director of the Team and/or Manager for administrative matters and is clinically accountable to the supervising physician(s).

The Physician Assistant practices in accordance with:

  • Delegation agreements and Medical Directives established with supervising physician(s) and at SAFHT
  • Regulated Health Professions Act, where applicable
  • Canadian Association of Physician Assistants (CAPA) Standards of Practice
  • Canadian Certified Physician Assistant (CCPA) professional expectations
  • SAFHT policies and procedures
  • Relevant Ministry of Health policies and clinical guidance

Qualifications

  • Graduate of a recognized Physician Assistant education program.
  • Successful completion of the Physician Assistant Entry-to-Practice Certification Examination.
  • Canadian Certified Physician Assistant (CCPA) designation, or eligible to obtain certification.
  • Membership in good standing with the College of Physicians and Surgeons of Ontario (CPSO).
  • Two years of recent clinical experience in primary care, family medicine, emergency medicine, internal medicine, or a related setting is preferred.
  • Demonstrated knowledge of comprehensive primary care across the lifespan.
  • Experience managing patients with chronic diseases using evidence-based clinical practice guidelines.
  • Strong history taking, physical examination, clinical reasoning, and diagnostic skills.
  • Demonstrated ability to work collaboratively within physician-led interdisciplinary teams.
  • Excellent communication, organizational, and interpersonal skills.
  • Current Basic Life Support (BLS) certification.
  • Commitment to evidence-informed practice, continuous learning, and quality improvement.

Assets

Preference may be given to candidates with one or more of the following:

  • Experience working in a Family Health Team, Community Health Centre, or academic family medicine clinic.
  • Experience managing complex patients with multiple chronic conditions.
  • Experience performing common office procedures independently within delegated authority.
  • Experience with preventive care initiatives and chronic disease management programs.
  • Experience using electronic medical records; proficiency with TELUS Practice Solutions Suite (PSS).
  • Experience participating in quality improvement, accreditation, and program evaluation.
  • Experience supervising or teaching students.
  • Ability to communicate in languages reflective of Scarborough's diverse communities.

Other Responsibilities

  • Provides comprehensive medical care under physician supervision in accordance with approved delegation agreements and Medical Directives.
  • Collaborates closely with supervising physicians to improve timely patient access and continuity of care.
  • Supports same-day and urgent access appointments.
  • Participates in preventive care recall initiatives and chronic disease follow-up.
  • Assists in planning, implementing, and evaluating health promotion initiatives.
  • Participates in interdisciplinary rounds, organizational committees, and quality improvement initiatives.
  • Maintains collaborative relationships with hospitals, specialists, Ontario Health Teams, and community agencies.
  • Maintains professional competence through continuing education and ongoing professional development.
  • Provides timely reports and statistical information as required.
  • Performs other duties as operationally required.
